- Notice of contract action. This is not a solicitation of offers. Under Statutory Authority 41 USC 253 (c)(1) and 38 USC 1720, the Government intends to enter into sole source acquisition negotiations for placement and services of eligible veterans into 8-nursing facilities located within the Northern Arizona VA Health Care System service area. This acquisition is for Community Nursing Homes within the Northern Arizona VA Health Care System service area, which includes Yavapai County, Conconino County, and Mohave County. The nursing homes identified are Legacy Home Healthcare, LLC, located at 3636 Crossings Dr, Prescott, AZ 86305, Havasu Nursing Center #2 Inc., 3576 Kearsage Dr, Lake Havasu City, AZ 86406, Health Care Management Professions, Inc., 3131 Western Ave, Kingman, AZ 86401, Kindred Healthcare Operating, Inc., 505 Jacks Canyon Rd, Sedona, AZ 86351, The Lingenfelter Center LTD., 1099 Sunrise Ave, Kingman, AZ 86401, Sandretto Hills Nursing Center, Inc., 1045 Sandretto Dr., Prescott, AZ 86305, The Evangelical Lutheran Good Samaritan Society, 3380 N Windsong Dr., Prescott Valley, AZ 86314, and Samaritan Village, Inc., 1030 Scott Drive, Prescott, AZ 86301. A Basic Ordering Agreement will be set up for a five year period with each facility. The authority cited and justification made are based on the determination that the Government's minimum needs can be best satisfied by services available from one source in a specific area of responsibility. Furthermore, in addition to being certified under Title XVIII or Title XIX, Nursing Home Agencies must meet specifications outlined in VHA Handbook 1143.2. This action is for services which the Government intends to solicit and negotiate with only the sources listed above under FAR 6.302-1. This notice of intent is not a request for competitive proposals. A determination by the Government not to open the requirement to competition based upon responses to this notice is solely within the discretion of the government. Information received as a result of the notice of intent will normally be considered solely for the purpose of determining whether to conduct a competitive acquisition.
