Description
INTENT TO PROCEED WITH PATIENT CENTERED MEDICAL HOME DEMONSTRATION LABORATORY COORDINATING CENTER VISN 20, VA Puget Sound Healthcare System (VAPSHC), North Branch Contracting, Tacoma WA, 98493 intends to negotiate a sole source Firm-Fixed Price contract and will utilize procedures under FAR Part 13, with Group Health, 1730 Minor Avenue Suite #1600, Seattle WA 98101. It is intended that this procurement will be sole source per 10 U.S.C. 2304 (c) (3) or 41 U.S.C. 253(c)(3) & FAR 6.302-3(a)(2)(ii), to establish or maintain an essential engineering, research or development capability to be provided by an educational or other nonprofit institution or a federally funded research and development center. A Veterans Health Administration (VHA) initiative is redesigning primary care around the Patient Centered Medical Home (PCMH) Model, a patient-driven, team-based approach that delivers efficient, comprehensive and continuous care through active communication and coordination of healthcare services. Most VHA Primary Care practices have already adopted many features of patient centered care and the medical home, but complete achievement will involve strategic assessment and redeployment of resources, realignment of priorities, and a cultural shift. This effort will be transformative and will align VA with national health care reform initiatives, enable us to continue to provide leadership in health care delivery, and assure that our Veterans' health continues to be managed with the highest quality, safety and effectiveness. VISN 20, VA Puget Sound Healthcare System (VAPSHC) intents to contract with Group Health Cooperative d/b/a Group Health Research Institute (GHRI) for the purpose of using the expertise gained by their staff in transitioning and adopting PCMH within the Group Health Cooperative care delivery system. Dr. Robert Reid, Ms. Katie Coleman and Ms. Kelly Ehrlich, designated staff from GHRI or other designees if staff changes occur during the time of the contract, will provide input and guidance to the PCMH CC in the development of performance and outcomes metrics and in reviewing ongoing progress and effectiveness of PCMH implementation. As the largest integrated healthcare system in the country, VA will benefit greatly from the experience of other health care systems that have implemented PCMH on a smaller scale and can provide tools and insights into effective metrics for measuring implementation and performance of PCMH. Because this is a new effort in VHA, there are inadequate internal resources/expertise to direct a project of this scope. VHA has funded five PCMH demonstration laboratories throughout the country. This contract is necessary to comply with the charge given to the PCMH CC by VHA's Undersecretary for Health and the Chief Primary Care Consultant. Because of the scope and high visibility of the project, it is imperative to our success that we engage the services of national organizations and experts in the area of Patient Centered Medical Home to guide our efforts. Dr. Stephan Fihn, Director of Northwest HSR&D Center of Excellence has been appointed by VHA Central Office to lead a PCMH Demonstration Laboratory Coordinating Center (PCMH CC) at VA Puget Sound Health Care System to 1) coordinate the demonstration laboratories and 2) to conduct the national evaluation of PCMH roll-out. This notice of intent is not a request for proposals or quotations. Information received will be considered solely for the purpose of determining whether conducting a competitive procurement is in the best interest of the Government in the future. Interested parties may identify their capabilities by responding to this requirement and considered for future reference. All information submitted should support the capability to provide the services needed, and shall be furnished at no cost or obligation to the Government. Responses must be in writing and must provide clear and concise documentation indicating a bona fide capability to provide the required service.
