Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Combined Synopsis/Solicitation no. W81K00-10-T-0282 is issued as a request for quotation (RFQ), subject to the availability of fiscal year (FY) 2011 funds. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-43. This acquisition is solicited on an unrestricted (full and open) competition basis for NAICS 325413. All responsible sources may submit a quotation that shall be considered by the agency. The requirements of this RFQ are for Anticoagulant Solution and Plasma Sampling Sets for Brooke Army Medical Center. Supplies are to be delivered monthly during the period of performance. Period of Performance: 01 October 2010 30 September 2011 Place of Performance: Fort Sam Houston TX 78234 FOB Point: Destination Line item 0001: ACD-A (Anticoagulant) Solution, 750 ML Bags Product/Part no: Caridian BCT Inc. 777-967-000 or Equal Quantity: 96 cases, 12 bags per case Required Delivery: 8 cases on or before the 15th of each month Unit Price: $____________________ per case (provide unit price quotation) Salient characteristics: Anticoagulant citrate dextrose sodium for use in the collection of red blood cells, platelets, and plasma on automated apheresis equipment. Also for use in the collection of platelets, plasma, and mononuclear cells, and for therapeutic apheresis procedures on epheresis equipment. Each 100 mL contains Dextrose Monohydrate USP: 2.45g, Sodium Citrate Dihydrate USP: 2.2g, Citric Acid Anhydrous USP 0.73g, in Water for Injection USP. Sterile, non-pyrogenic for use only with automated apheresis devices. Single use bags, 750mL total volume. Must be compatible with Trima (registered) automated blood collection system and COBE (registered) Spectra (trademark) apheresis system. Set includes pre-attached sampling system with needle and bottle adapter for transfer of sample to the culture media. 10mL reservoir with 0.5mL increments for accurate delivery of sample. Self-aligning bottle adaptor enabling fail-safe inoculation. Flow control for accurate sample transfer. Protection layer for user safety. The salient physical, functional, or performance characteristics that equal products must meet are specified in the solicitation. (b) To be considered for award, offers of equal products, including equal products of the brand name manufacturer, must (1) Meet the salient physical, functional, or performance characteristic specified in this solicitation; (2) Clearly identify the item by (i) Brand name, if any; and (ii) Make or model number; (3) Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and (4) Clearly describe any modification the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modification. (c) The Contracting Officer will evaluate equal products on the basis of information furnished by the offeror or identified in the offer and reasonably available to the Contracting Officer. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. (d) Unless the offeror clearly indicates in its offer that the product being offered is an equal product, the offeror shall provide the brand name product referenced in the solicitation.
