Description
The Government intends to evaluate offers and award a contract without discussions with Offerors providing the requested product. Therefore, the initial offer should contain the offerors best terms from a technical and price standpoint. The North American Industry Classification (NAICS) Code is 611430 and the Size Standard is $7,000,000 for this requirement. The Division Healthcare Quality Promotion maintains the National Healthcare Surveillance Network (NHSN). NHSH collects data from a sample of healthcare facilities in the United States to permit valid estimation of the magnitude of adverse events among patients and healthcare personnel. The Hemovigilance Module is a component of NHSN and was released February 2010 for transfusion services with NHSN participating hospitals to report recipient adverse reactions and quality control incidents related to blood transfusion. this project is to provide a one-day training to NHSN Hemovigilance Module users on how to use the NHSN survailance system. This training will be offered in conjunction with the 2010 AABB Annual Meeting where users from throughout the US will be in attendance. Individually, and not as an agent of the Government, the Contractor will furnish all labor, materials, equipment and meeting space required to provide a one-day training to approximately 100 NHSN users who will be in attendance at the AABB Annual Meeting. The vendor shall perform the following tasks: Task 1: Pre-Training Requirements • Within one week of the effective date of the Purchase Order, the Contractor shall arrange an initial conference call with the Project Officer to review the proposed training agenda, delineate roles and responsibilities, and establish communication protocols. • As a result of the meeting, the Contractor will submit a final draft agenda and drafts of required hand-outs to the Projecft Officer no later than two weeks following the meeting. • The Government will review the draft report and provide comments for completion of a final NHSN Hemovigilance Training Agenda and hand-outs within 2 weeks of receipt from the Contractor. • The Government will be responsible for dessimination of training information and registration of approximately100 participants. A list of registered attendees will be provided to the Contractor no later than October 1, 2010. Task 2: Training Requirements • Training will occur on October 7, 2010 at Baltimore Convention Center • Provide a meeting room set-up for lecture-style presentation that will accommodate a minimum of 100 participates. • Provide A/V support for the training. • Will provide all hand-outs required for the training. • Will distribute and collect training evaluation forms. Task 3: Post-Training Requirements Within 4 weeks following completion of the training, the Contractor will provide CDC with: • List of attendees, including name of organization/state of the attendee. • Summary of training evaluations submitted by attendees. • Post-training teleconference to discuss participant feedback and potential revisions for future training sessions. The period of performance shall commence on the date of receipt of the purchase order and shall continue through November 10, 2010. The vendor will provide the following deliverables to the Project Officer by the dates required. TASK# TASK ACTIVITY DELIVERABLE DELIVERY DATE Task 1 Develop Training Agenda and Hand-outs Draft Training Agenda and draft hand-outs Within 3 weeks of task order award Task 1 Registration of Attendees List of Attendees 10/4/2010 Task 3 Summary of Training Final List of Attendees and Summary of evaluations 11/10/2010 This requirement is not a small business set-aside. Interested vendors capable of furnishing the government with the service in this combined synopsis-solicitation should submit an electronic copy of a there proposal to the below address. The quotation shall include the itemized price and descriptive literature of the product quoted. The quotation must reference RFQ number 88226. Quotations will be due fifteen (10) calendar days from the publication date of this synopsis or August 30, 2010, by 11:30 a.m. EST time. This action is pursuant to 41 USC 253(1) and under the authority of FAR 6.302-1. The requirement will be procured as a commercial item. A determination by the Government not to compete this proposed requirement based on responses to this notice is solely within the discretion of the Government. THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. The following provisions apply to this acquisition; 52.212-1 Instructions to Offerors - Commercials Items (MAR 2000) applies to this acquisition; 52.212-2, Evaluation - Commercial Items (JAN 1999); the evaluation criteria stated in paragraph (a) of this provision are as follows: (1) technical capability of the proposed system to meet the requirements stated herein, and (2) price. The clause 52.212-4, Contract Terms & Conditions - Commercial Items (MAY 1999) applies to this acquisition. The following FAR Clauses are hereby added to this clause: 52.247-35 F.O.B. Destination; The clause at 52.212-5 - Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items applies to this acquisition. The following clauses listed in paragraph (b) of 52.212-5 apply to this acquisition: (1), (3), (5), (7), (9), (11), (12), (14), (15), (17), (18), (20) and (24). All responsible sources may submit a quotation, which if timely received, shall be considered by the agency. Quotations must be submitted in writing to glm5@cdc.gov, attention: Gary Morgan. No phone calls will be addressed. Any interested parties that believes it can meet the above requirements, may submit a statement of capabilities. The capability statement and any other furnished information must be in writing and must contain material in sufficient detail to allow CDC to determine if the party can fully meet the requirements herein.
