Description
This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number VA-262-10-RQ-0453 is issued as a request for quote (RFQ) and is a brand name or equal. Brand name or equal description is intended to indicate the quality and characteristic of products that will be considered satisfactory to meet the agency requirements. No telephone requests will be accepted. This solicitation incorporates provisions and clauses as those in effect through Federal Acquisition Circular 2005-43. The North American Industry Classification System Code (NAICS) is 423440 and the Small Business Standard is 100 employees. Line Item 01: Evacuation Chair 1 EA - UNIT PRICE $ _______________ TOTAL PRICE FOR 10 CHAIRS: $ _______________ Stryker Evacuation Chair - Model 6253 Height 45" (114 cm) Width 20.5" (52 cm) Depth3 28" (71 cm) Folded Depth 11" (28 cm) Weight 34 lbs (15 kg) Maximum Load 500 lbs (227 kg) Delivery of equipment shall be F.O.B. Destination to the VA San Diego Healthcare System, 3350 La Jolla Village Dr., San Diego, CA 92161, within consignee's premises. At the time of delivery, the equipment and all associated equipment shall be new, free from defects and in good working condition. Re-manufactured equipment will not be accepted. Offerors must comply with all instructions contained in provision 52.212-1 Instructions to Offerors - Commercial Items. Pursuant to FAR Clause 52.212-4, para (t), Central Contractor Registration (CCR), after initial registration, the contractor is required to update registration data as changes occur and must re-register annually to ensure all data remains current. Noncompliance with this requirement will preclude the exercising of any option periods that may be included herein and will be cause for termination of the contract at such time noncompliance is discovered. It is imperative that you complete this registration in order to be considered for contract award.
