Description
The Great Lakes Acquisition Center (GLAC) in Milwaukee, WI intends to negotiate on a sole source basis (IAW FAR 13) with Mizuho Orthopedic Systems Incorporated, 30031 Ahern AVE, Union City, CA 94587-1234 as the only source that can provide an orthopedic trauma top which can be used with the modular OR table base system already owned by Hines VA. Specifically, the contractor shall provide the following: 1 - Orthopedic Trauma Top 500 LB Capacity, (Part #5855, Quantity 1); 2 - Pivoting Arm Board, (Part #5356, Quantity 2); 3 - Cross Arm Support, (Part #5857, Quantity 1); 4 - Orthopedic Equipment Cart (Part #5859, Quantity 1); 5 - 6 piece Tempur MED Positioning Pad Set (Part #6950, Quantity 1); 6 - Traction Boots W/Swivel Plates, 1 PR, X Large (Part #5855-862, Quantity 1); 7 - Universal Side Rail Adaptor (Part #6977-959, Quantity 2); This acquisition is being conducted under simplified acquisition procedures (IAW FAR 13). There are no set-aside restrictions for this requirement. The intended procurement will be classified under North American Industry Classification System (NAICS) 339112 with a Small Business Size Standard of 500 employees. This notice of intent is not a request for competitive proposals and no solicitation document exists for this requirement. However, parties interested in responding to this notice will need to submit technical data sufficient to determine capability in providing the same product. All capability statements received within 5 days after publication of this synopsis will be considered by the Government. A determination by the Government not to compete based on responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. Capability statements shall be submitted by email only as a MS Word or Adobe PDF attachment to Scott Ivy at scott.ivy@va.gov Statements are due by 2:00 CST 01 Sep 2010. No phone calls accepted.
