- The G.V. Sonny VA Medical Center, Jackson, Mississippi has a requirement for a non-personal service contract for an Adult Day Health Care Facility. CADHCF will be furnished to beneficiaries for whom such care is specifically authorized by the G.V. (Sonny) Montgomery VA Medical Center. Veterans served shall fall in the catchment area - map attached. It is understood that the type of patient to be cared for under this basic ordering agreement (contract) will receive these services as a substitute for institutionalization. The CADHCF must be within 100 mile radius of the G.V. Sonny Montgomery VA Medical Center in Jackson, Mississippi. The CADHCF program has five basic purposes: a. To enable functionally impaired veterans to reside in supportive home environments. b. To facilitate expeditious medical center discharge and to reduce risk of readmission of institutional placement through improved provision and coordination of health care services and education of patients and care givers about service options and their appropriate use. c. To maximize the veteran's physical and psychosocial functional level. d. To improve the quality of life for the participants by providing a rehabilitation program in the community among their peers. e. To provide support and respite for the family and other care givers to enable them to maintain veterans in the community. The G.V. Sonny VA Medical Center intends to award a basic ordering agreement firm fixed price contract to the successful offeror whose proposal presents the best value to the Government considering technical capabilities and past performance of the contractor and price. The resultant contract will be for a base period of one year with four additional one year option periods. This solicitation will be set aside 100% for small businesses. The NAICS code identified for this solicitation is 624120, services for the elderly and persons with disabilities, and the small business size standard is $7.0 million. The solicitation will be issued on or around August 10, 2010 and proposals will be due to the Contracting Officer by 3:00 P.M. on Friday, September 3, 2010.
