- Attachment III Cost Price Worksheet (97- 2003) Attachment III Cost Price Worksheet (2010) RFQ, Attachment I SOW, and Attachment II: Proposal Submital Instructions and Evaluation Federal Emergency Management Agency Office of External Affairs, Office of Disability Integration and Coordination Conference is hereby issuing a Request For Quotation (RFQ) in anticipation of awarding a Firm-Fixed Price (FFP) Purchase Order (PO) to the hotel offering the best value to the Government in response to the services outlined in the schedule & Statement of Work(SOW). FEMA's Office of Disability Integration and Coordination with Citizens Corps, the National Protection and Preparedness Directorate and the Interagency Coordinating Council on Emergency Preparedness and Individuals with Disabilities will host a three-day Inclusive Emergency Management National Capacity Building conference. Conference to be held September 22nd through September 24th, 2010. Attendees arrive on September 21st. Questions shall be submitted via email by 12PM EST on Monday August 16, 2010 to Marcelle.Loveday@associates.dhs.gov. (No phone calls please) Quotations shall be submitted via email by 12PM EST on Wednesday August 18, 2010 to Marcelle.Loveday@associates.dhs.gov. (See attachment for full Request for Quotations including SOW, Proposal Submital Instructions, Evaluation Factors, and Provisions and Clauses). An excel spreadsheet in Microsoft 97- 2003 and 2007 has been provided for cost/price proposal submissions (See Attachment Three).
