Description
This is a notice of intent, not a request for a proposal. A solicitation document will not be issued and proposals will not be requested. The National Institute on Drug Abuse (NIDA), Station Support/Simplified Acquisition (SS/SA) Branch, on behalf of the National Institute on Mental Health (NIMH) intends to solicit, negotiate and award a purchase order on a noncompetitive basis with the World Health Organization (WHO) in Geneva, Switzerland for the establishment and maintenance of a mhGAP electronic reference library and the development of normative tools to assist in the implementation of mhGAP in countries. Mental, neurological, and substance use (MNS) disorders are prevalent in all regions of the world and are major contributors to morbidity and premature mortality. The resources to tackle the huge burden are insufficient, inequitably distributed, and inefficiently used. The result is a large treatment gap, more than 75% in many low and middle income countries (LAMIC). Mental ill-health and poverty interact in a negative cycle: mental ill-health impeded people's ability to learn and to engage productively in their economies, and poverty in turn increases the risk for developing mental disorders, and reduces people's ability to gain access to health services. In order to respond to the growing challenge and to scale up care for MNS disorders in LAMIC, the World Health Organization (WHO) has developed the mental health Gap Action Programme (mhGAP) which was launched by the Director-General of WHO in October 2008. Scaling up strategy as described in mhGAP focuses on ways to increase the impact of health service interventions to benefit more people and to foster policy and programme development on a lasting, sustainable basis. NIMH requests the following activities to be conducted by the WHO: Scope of Work 1) Establish and maintain a mhGAP electronic reference library The purposes of the electronic library are to provide easy access to 1) the best clinical and non-clinical information on evidence-based interventions for MNS disorders, 2) best practices relevant to mental health and social care for treatment decision-makers in varied care settings, and 3) a reference library on scaling up successful intervention in low- and middle-income countries. The WHO will develop, maintain, and update a web-based virtual library for health professionals, accessible using any internet browser and linked with other web-based sources of evidence-based information, including evidence reviews carried out for the development of the mhGAP intervention package. The resource library will provide the latest information on clinical decision-making for management of MNS disorders in non-specialized health care settings in low and middle income countries. The WHO will assemble content experts to compile scientific and gray literature on scale-up of interventions, review and synthesize the literature, identify gaps in research, and design recommendations to assist countries and researchers in focusing their efforts on implementation and scale-up of interventions. 2) Develop normative tools to assist implementation of mhGAP in countries Before the implementation of mhGAP, the intervention package will need to be adapted according to national policies, needs and resources. The WHO will develop the mhGAP intervention package adaptation guide. The guide should describe the process for countries to use to adapt the generic evidence based guidelines for the care of patients who come to first or second level health facilities. The generic guidelines will be adapted to ensure that 1) they address the conditions that contribute most to the burden of MNS disorders in a specific country and 2) that they are appropriate for the local conditions that affect the care of patients with mental disorders in the health facility. The guide will assist teams in making decisions about what adaptations are needed and in completing the tasks required to produce adapted materials for training non-specialist health care providers. The WHO will develop training materials for the different cadres of health planners and non-specialist care providers. The materials should develop the capacity of health planners to systematically plan, implement, coordinate and monitor the delivery of care and services for people with MNS disorders through existing health systems. WHO will develop training materials for specialists and non-specialist providers. The specialists would be trained to become "Master trainers" and material would need to be developed for both specialists and non-specialists. The non-specialist health care providers include general physicians, primary care doctors, nurses, clinical officers and others. The WHO will also field test materials before using it in countries. Monitoring and evaluation tool: The WHO will develop monitoring and evaluation tools for planning and implementation of the program. These will include the development of guidelines that describe the scope of monitoring and evaluation, process of identification and selection of indicators and identification of tools and methods of measurement for implementers. The indicators for measurement will include program inputs and activities, program outputs, outcomes, and impact/health status. The tool should also assist countries in 1) developing the plan for monitoring and implementation; 2) identifying which indicators to measure and for what purpose, when and where to measure them, how to measure them, which data sources to use; and 2) planning for analysis and use of the data. The WHO is the international public health agency with years of experience working to improve health and living standards for people around the world. They conducted the original assessments of the status of 190 countries' policies, services, resources, and personnel in mental health and published and widely distributed this information in a document titled, Atlas: Country Profiles on Mental Health Resources (2001). In addition, they have already identified collaborators in designated countries to work with them on this project and have received support for the project from both the WHO organization and Council and the Ministers of Health. They have both the credibility and expertise to carry out this project in all six WHO regions of the world. The period of performance is 2 years with the option of one additional 12-month period. The place of performance is primarily Geneva, Switzerland. Therefore based on the aforementioned facts, only one responsible source and no other supplies or services will satisfy agency requirements FAR 6.302-1. The Government intends to select and evaluate and award the offer that represents the best value to the Government, price and all other factors considered. The acquisition is being conducted under simplified acquisition FAR 12 and is exempt from the requirements of FAR Part 6, Competition Requirement. This notice of intent is not a request for competitive proposals. Interested parties may identify their interest and capabilities in response to this synopsis. The determination by the Government not to compete the proposed contract based upon responses to this notice is solely with the discretion of the Government.
