Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, in conjunction with FAR 13.5, as applicable, and as supplemented with additional information included in this notice. This solicitation is issued as a Request for Proposal (RFP); solicitation number is VA-248-10-RP-0470. This announcement constitutes the only solicitation and a written solicitation will not be issued. The North American Industry Classification System (NAICS) code is 561720 with a size standard of $16.5 million. This procurement is 100 % SET ASIDE FOR HUBZone CERTIFIED SMALL BUSINESSES. All prospective offerors must be actively registered in the List of Qualified HUBZone Small Business Concerns on its Internet website at http://www.sba.gov/hubzone Offerors shall also be registered on the Central Contractor Registry (CCR) and ORCA (Online Representations and Certifications Application. Offerors may register online at www.ccr.gov and on https://orca.bpn.gov. Lack of registration in CCR and ORCA will qualify the contractor as ineligible for award. The solicitation incorporates provisions and clauses in effect through the Federal Acquisition Circular (FAC) 2005-44. GENERAL STATEMENT OF WORK: 1. The contractor shall furnish all necessary labor, equipment, supplies, and supervision to render complete Janitorial Services at the VA Caribbean Healthcare System, Medical Center Building, 1ST floor, 4th to 9th floor, including VA District Counsel Building, VA Outpatient Clinic Building (OPA), VA Nursing Home Care Unit Building (VA NHCU), located at Calle Casia, San Juan, Puerto Rico 00921, VA Mayaguez Outpatient Clinic (MOPC), located at Avenida De Hostos State Road #2, Mayaguez, Puerto Rico 00680 and VA Ponce Outpatient Clinic (POPC), located at Ave. Santiago de los Caballeros, Ponce, Puerto Rico 00731, identified in this solicitation. Also the offeror agrees to provide the aforementioned services, in other VA Caribbean Healthcare System facilities located in Puerto Rico, upon written supplemental agreement (if necessary). 1.2 Term of Contract: The period of performance shall begin on October 1st, 2010 through September 30, 2011. 1.3Type of Contract: VA anticipates award of a Firm-Fixed Price contract based on the contents of this solicitation. 2. LOCATIONS: 2.1VA Caribbean Healthcare System (VACHS) Medical Center Building (MC) First Floor. And VA District Counsel Building (VADC). 10 Calle Casia San Juan, Puerto Rico 00921-3201 2.2 VA Out-Patient Additional Building (OPA) 10 Calle Casia San Juan, Puerto Rico 00921-3201 2.3 VA Nursing Home Care Unit (NHCU) 10 Calle Casia San Juan, Puerto Rico 00921-3201 2.4 VA Mayaguez Outpatient Clinic (MOPC) Avenida De Hostos State Road #2 Km. 156.2 Mayagüez, Puerto Rico 00680-1507 2.5 VA Ponce Outpatient Clinic (POPC) Lot #37 Ave. Santiago de los Caballeros Finca Multedo Estrella State Road PR #14, Km 3.3 Ponce, Puerto Rico 00731 3. BUILDING AREAS: 3.1 The Government occupied space consists of the following net usable square feet: 3.1a VACHS MC 1st floor 173,000 sq. ft. 3.1b VACHS MC 4th-9th floor 132,000 sq. ft. 3.1c VA OPA 160,000 sq. ft. 3.1d VA NHCU 65,090 sq. ft. 3.1e VA MOPC 54,090 sq. ft. 3.1f VA POPC 55,720 sq. ft. 3.1g Other VA locations may be incorporated after contract award upon bilateral agreement between the (Contractor) and the Government. At present the new VA South Bed Tower haven t been activated. Upon knowledge of an opening date complete custodial services for the South Bed Tower (SBT) might be incorporated in the contract via supplemental agreement. Estimate amount of SQ. Ft. at the SBT are 228,000. 4. BUILDING OFFICE HOURS: 4.1 The VA Caribbean Healthcare System, Medical Center regular office working hours will be Monday through Friday from 7:00 AM to 4:30 PM. Regular working hours for Mayaguez and Ponce are the same as the VACHS. VACHS is occupied twenty four (24) hours per day, seven (7) days per week. Performance scheduled to be arranged by the delegated Contracting Office Technical Representative (COTR). 5. SUPERVISION: 5.1The Contractor shall arrange for satisfactory supervision of the contract work. The Contractor and/or his Supervisors shall be available at all times, when the contract work is in progress, to receive notices, reports or requests from the Contracting Officer and/or COTR. The Contractor shall provide in writing to the Contracting Officer the name, telephone number and address of on-site supervision at least five working days prior to the contract starting date. 6. COMPETENCY REQUIREMENTS: 6.1The contractor shall comply with Joint Commission Accreditation of Health Care Organizations (JCAHO) requirements, to comply with JCAHO requirement the contractor shall provide written confirmation that employees are competent to perform the task as described in the statement of work assigned on this contract. This written confirmation shall specify in which areas each contract employees are competent. Upon contract award-the Contracting officer and/or COTR will provide a Contract Employees handbook for contract references. 7. SECURITY CLEARANCE: 7.1The contractor shall submit to the COTR the following: Certificate of Good Conduct from the Police Department and, Statement of Personal History , Employee Health Certificate from the Department of Health of all employees who have access to the building in performance of the contract work at least five working days prior to the starting date of contract. Contractor employees shall have a physical examination prior to work on this contract and annually thereafter. The contractor shall assure that the each contracted employee meets the requirements as it relates to physical examinations : PPD skin testing, Hepatitis B, prophylaxis s or declination of option signed and on file, other preventive health screening/testing as required by OSHA(Occupational Safety and Health Administration, JCAHO, Joint Commission on Accreditation of Healthcare Organizations and other screening agencies. Contractor shall comply with all applicable Federal, State, local and industry safety and health standards and regulations 8. SUPPLIES, MATERIALS AND EQUIPMENT: 8.1 All utilities (electrical and water) will be furnished by VA for the Contractor s use to operate such equipment as is necessary in the conduct of his work. 8.2 The Government will provide the space at each building to the Contractor upon availability for the storage of an inventory supplies and equipment that be used in the performance of work under the contract. The contractor shall maintain this space in a neat, clean and orderly condition. 8.4 The Contractor shall furnish all supplies, materials and equipment necessary for the performance of the work of this contract, including but not limited to: brooms; brushes; cleaning cloths; treated and untreated dusting cloths; wet mops; dust mops; sponges; squeegees; paper towels; paper towel dispensers; toilet tissue; toilet tissue dispensers; hand soap (medicated and non- medicated); hand soap dispenser; porcelain cleaner; glass cleaner; paper bag liners for sanitary napkin disposal receptacles; detergents; floor finish; floor stripper; metal polish; furniture polish; scouring powder, germicidal detergent; plastic bag liners (clear and red) for trash cans required; air freshness. 9. QUALIFICATIONS OF CONTRACTOR PERSONNEL: 9.1 The personnel employed by the contractor shall be capable employees, qualified in janitorial services type work with at least one (1) year of medical facility cleaning experience. 9.2 The building shall be fully staffed beginning the first day of work all staff shall be fully trained and experienced cleaning personnel who shall exhibit the capability of operating with a minimum of supervision. 10. QUALITY CONTROL PROGRAM: 10.1 The contractor shall establish complete quality control programs to ensure the requirement of the contract are provided as specified.
