Description
(i)This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in subpart 12.6, in conjunction with the policies and procedures for solicitation, evaluation, and award as prescribed under FAR 13.1, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will NOT be issued. Under Statutory Authority 41 U.S.C. 253(c)(1) the Minneapolis VA Medical Center (VAMC) anticipates entering into a sole source procurement with St. Jude Medical, Inc. of Saint Paul, MN for EnSite Velocity Cardiac Mapping System Upgrade The government believes that St. Jude Medical, Inc is the only manufacturer that will satisfy the agency requirements (ii) The solicitation number is VA263-10-RQ-0527 and is issued as a Request For Quotation (RFQ). (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-43, July 23, 2010. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at http://www.arnet.gov/far (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR). (iv). The North American Industry Classification System (NAICS) code is 334510, and the size standard is 500 employees. (v) This requirement consists of four (4) line items: CLIN (1): EnSite Velocity Cardiac Mapping System upgrade, to include operator and service manuals. Quantity 1; CLIN (2) EnSite Fusion Registration Module, Quantity (1); CLIN (3) EnSite Velocity RecordConnect and additional hardware module, Quantity (1); CLIN (4) EnSite Velocity GenConnect and additional hardware module, Quantity (1) (vi) The EnSite Voleocity System upgrade shall contain: anew DWS5 display workstation (most current version), amplifier, NavLink, ArrayLink, optional mobile carts, two 24 high definition widescreen flat panel monitors. Also a GenConnect interface module to connect to a 1B11500T9 ablation generator. A GenConnect interface module to connect to an alternative manufacture s ablation generator. A RecordConnect cable interface. A one year assurance plus customer protection plan. The most current cersion of software. A EnsSite connect remote access for technical support. T his solicitation will require the inclusion of security language from VA Handbook 6500.6, located at https://www.voa.va.gov/DocumentView.aspx?DocumentID=228. The awardee will be required to agree with and act upon the following sections of that handbook: 1. General, 2. Access to VA Information and VA information Systems, 4. Information system design and development-paragraphs (c,d,e,g), 5. Information System Hosting, Operation, Maintenance, or Use paragraphs (,h), 6. Security Incident Investigation, 9. Training. The procurement of these items will require the awardee to sign a Business Associate Agreement (BAA). It will also require a low risk (NACI) background investigation for all contracted employees working on site at the MVAHCS. The offer shall specify technical specifications and proposed adaptive equipment and provide estimated number of days to deliver. (vii) Delivery FOB Destination to VA Medical Center, One Veterans Drive, Minneapolis, MN 55417. The Government will award a firm fixed price contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, technical factors combined with past performance is equal to price.
