- Performance Work Statement NELLIS AFB CATHOLIC AUXILIARY PRIEST SCOPE OF WORK: Environment: 1. Nellis Air Force Base is a military community supporting Air Combat Command at home stations and in many theater, region, or contingency areas as part of any force, joint or allied, in support of national objectives. 2. Mission changes, consolidation of mission activities, increased operation tempo and rapidly shrinking resources facilitates a high degree of community fragmentation that impacts the military infrastructure and makes maintaining chapel programs a challenge. Characteristics of Auxiliary Catholic Priest: 1. Within the above-described parameters this Performance Work Statement (PWS) addresses one of the identified critical needs and will be designed to provide for an Auxiliary Catholic Priest for the Nellis Air Force Base Chapel. 2. To function in the capacity of an Auxiliary Catholic Priest during the period of 01 October 2010 through 30 Sep 2015. QUALIFICATIONS: Positive, spiritually sensitive clergy member who specializes in working with chapel staff members, all military personnel and chapel volunteer lay leaders, with these specific criteria: a. Must be a Roman Catholic priest in good standing with the Catholic Church. b. The Catholic Chaplain and/or Wing Chaplain will interview the potential contractor prior to award. 1. Must be a validly ordained priest and have the endorsement of the Military Archdiecese to work and be granted unrestricted faculties to function as a priest by the Military Archdiocese. 2. Must furnish written proof thereof at time of interview. 3. Must have no criminal record and be willing to provide a background check. Character: Conctractor must be positive, self-directed, motivated, and able to interface well with people. Will maintain high standards of personal and moral conduct and decorum expected of all those who represent the chapel, including but not limited to dress and grooming. The ability to work collaboratively with a diverse chapel staff is a must and will be adhered to. SKILLS: Contractor must possess strong people/social skills in order to help build and lead a Catholic Community. The ability to work with various chapel committees and groups is crucial. The ability to listen and follow instructions from the Wing Chaplain, Non-Commissioned Officer In Charge (NCOIC) and Chapel Operations is necessary. DATES, TIMES, AND PLACES OF PERFORMANCE: The contractor shall conduct daily Masses, Holy Day Masses, and provide funeral, and memorial services, etc. during the fiscal year(s) as needed. Rite of Reconciliation is Saturday at 1600. Saturday Mass is at 1700. Sunday Masses are at 0945-1030 and 1230-1330. The Reconciliation, as well as weekend Mass schedules are not negotiable. Specific work schedule will be coordinated through the Catholic Chaplain or the Wing chaplain. The Chapel, building 4302 N Washington Blvd, Nellis AFB, NV will be the primary place for all services unless otherwise directed by the supervising chaplain. CONTRACTOR RESPONSIBILITIES: The Contractor shall agree to furnish the following described services at the locations, intervals and times specified by the Nellis Chapel: 1. Conduct religious worship and instruction services, devotions, and liturgical rites as required. Contractor will conduct the regularly scheduled weekend masses at a minimum of once a month. 2. Services will be contracted on a per unit basis. A unit is defined as: a. Weekday Mass - performed Tuesday thru Friday beginning at 1130. Mass lasts approximately 30 minutes. b. Weekend Mass - performed Saturday at 1700. Sunday at 0945 and 1230. Mass lasts approximately 1 hour. c. Other - Liturgical Rites including but not limited to Rite of Reconciliation (Confession), Funerals/Memorial services, Celebration of the Sacrements Baptism, Confirmation, Holy Eucharist and Marriage Ceremonies. Services performed as requested. 3. Contractor shall adhere to Nellis Air Force Base Operating Instructions (OI's) and 52 Series Air Force Instructions (AFI's) when using Chapel facilities. 4. Contractor is permitted to provide subcontractors who are equally qualified and when prior approval is given by the Catholic Chaplain or Wing Chaplain. PAYMENT PROCEDURES: The contractor will be responsible for filing any applicable social security, federal, or state taxes. Contractor will be required to submit a monthly invoice before payment is made. TERMS AND CONDITIONS 1. DESCRIPTION OF AGREEMENT: This is a pre-priced Blanket Purchase Agreement (BPA) for non-personal services. Under this BPA, the contractor shall perform all operations in connection with providing Catholic Religious Services to Nellis Air Force Base. These services include but are not limited to Weekday Mass, Weekend Mass, Rite of Reconciliation, Funerals and Memorial Services. These services will be ordered by the Catholic Chaplain and/or Wing Chaplain (or authorized representative of the Chaplain) during the five (5) year duration of this Blanket Purchase Agreement during the period, 01 October 2010 to 30 September 2015. The Chapel, building 4302 North Washington Blvd, Nellis AFB NV 89191 will be the primary place for all services unless otherwise directed by the supervising chaplain. 2. EXTENT OF OBLIGATION: The government will be obligated only to the extent of authorized purchases actually made under this BPA by authorized personnel. 3. PRICING: Prices quoted for the religious services shall be as low as or lower than those charged under the supplier's most favored customer for comparable quantities under similar terms and conditions, in addition to any discounts for prompt payment. 4. PURCHASE LIMITATIONS: Individual Call Orders placed under this BPA will not exceed a total combination of $2,000.00 per call. 5. NOTICE OF INDIVIDUALS AUTHORIZED TO PURCHASE UNDER THIS BPA AND DOLLAR LIMITATIONS: A list of names of individuals authorized to purchase under this agreement, identified by title of position and by name of individual, organizational component, and the dollar limitation per purchase. 6. DELIVERY TICKETS: An original delivery ticket or sales slip shall accompany all deliveries or shipments under this BPA. The delivery ticket or sales slip shall contain the following minimum information. A. Name of Supplier. B. BPA Number. C. Date of Purchase. D. Purchase number. E. Itemized list of supplies or services furnished. F. Quantity, unit price, and extension of each item, less applicable discounts. G. Date of delivery of shipment. 7. PRICE LIST. Price lists are subject to the following terms and conditions: A. Price Lists on company stationery or letterhead signed as original copy will be submitted to the contracting officer for approval. The Price List at a minimum, will include the flat rate price for providing the following services: WEEKDAY MASS WEEKEND MASS RITE OF RECONCILIATION FUNERALS /MEMORIAL SERVICE CELEBRATION OF THE SACREMENTS BAPTISM CONFIRMATION HOLY EUCHARIST & MARRIAGE CEREMONIES B. Contractor is authorized to submit an annual change to the price list. Change must be submitted to the Contracting Officer no later than thirty calendar days prior to the effective date of the price change. Any price list changes other than the annual request must be submitted with adequate justification for price increase. C. The contractor may offer voluntary price reductions (VPR) for previously approved items without written approval. These reductions should be shown as "VPR" on the invoice. 8. WORK REQUESTS. Orders under this BPA may be made orally by any appointed Call Order Agent. Work performed or services provided, may be paid only by third party government purchase credit card. 9. INVOICE. A. An itemized invoice shall be submitted at least monthly or upon expiration of this BPA, whichever occurs first, for all services rendered during a billing period and for which payment has not been received. Copies of delivery tickets need not support these invoices. B. Invoices signed by the contractor and in original copy, shall be sent to: NELLIS CHAPEL 4302 NORTH WASHINGTON BLVD NELLIS AFB NV 89191 C. The period of any discounts will commence on the final date of the billing period or on the date of receipt of invoices for all services rendered during the billing period, whichever is later. 10. SHIP TO. N/A 11. DISCOUNT TERMS. As stated on individual invoices. 12. PAYMENTS. Payments will be made by: GOVERNMENT PURCHASE CREDIT CARD 13. COMPLIANCE WITH APPLICABLE FEDERAL, STATE AND LOCAL REQUIREMENTS. In performing work under this agreement, the Contractor shall comply with all relevant federal, state and local statutes, ordinances, laws and regulations. 15. PROVISIONS ON CONTRACTING OFFICERS AUTHORITY The Contracting Officer is the only person authorized to approve changes in any of the requirements under this agreement, and notwithstanding any provision contained elsewhere in this agreement, said authority remains solely with the Contracting Officer. In the event the contractor effects any such change at the direction of any person other than the Contracting Officer, the change will be considered to have been made without authority and no adjustment will be made in the contract price to cover any increase in costs incurred as a result thereof. 16. CANCELLATION: Subject BPA is cancelable, by either party, with a thirty (30) day written notice.
