- WAGE DETERMINATION STATEMENT OF WORK SCHEDULE B - CONTRACT LINE ITEMS REPRESENTATIONS & CERTIFICATIONS PAST PERFORMANCE QUESTIONAIRE EXHIBIT A - FRAMEWORK CLAUSES & EVALUATION CRITERIA This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are requested and a written solicitation will not be issued. The solicitation number is AG-3143-S-10-0001 is issued as a request for proposal RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-43. The associated North American Industrial Classification System (NAICS) code is 541199 with a small business size standard of $7.0M. This requirement is unrestricted and only qualified offerors may submit proposals. The list of contract line items is contained within this notice. The period of performance is anticipated to be a Base Period and four (4) Optional Periods. Offerors please read entire combined/synopsis solicitation package. The U.S. Department of Agriculture (USDA) requires Claims Administration and Adjudication services to resolve discrimination allegations by female and Hispanic farmers and ranchers. A detailed statement of work is contained within this notice. Offerors must be registered in the Central Contractor Registration (CCR) database before an award can be made to them. If the offeror is not registered in the CCR, please do so through the CCR website at www.ccr.gov. The selected offeror must comply with the following commercial item terms and conditions, which are incorporated within this notice. FAR 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition: FAR 52.212-2, Evaluation-Commercial Items and evaluation factors are included within this clause and this clause applies to this acquisition; FAR 52.212-3, Offerors Representations and Certifications-Commercial Items-the all offerors must submit a completed copy of the listed representations and certifications, FAR 52.212-4, Contract Terms and Conditions-Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, paragraphs (a) and the following clauses in paragraph (b): 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-41, 52.232-33.
