- The Department of Veterans Affairs, Iowa City VA Medical Center, Iowa City, IA intends to negotiate a sole source procurement under the authority of FAR 6.302.1, with The University of Iowa 4D Jessup Hall, Iowa City, IA 52242-1316. A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA-263-10-RQ-0591 and issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-35, effective July 14, 2009. The associated NAICS code is 611310 and small business size standard is 7.0M. This is not set-aside and is being negotiated as a sole source procurement. The requirement for this acquisition includes the following: ITEM QTY UNIT UNIT COST TOTAL COST ______________________________ 1 College staff will provide research trial pharmacist services to the Center for Research in the Implementation of Innovative Strategies in Practice (CRIISP) at the Iowa City VA Medical Center. This college has previously worked on research involving medication adherence and blood pressure monitoring. The departmental knowledge and expertise of this research and the previous work with Drs. Rosenthal and Carter on the currently funded VA grant investigating high and low intensity strategies for blood pressure control will significantly increase the impact of this grant. I. The primary tasks are as follows: a. Conduct detailed patients interviews to evaluate current treatment strategies and patient factors that may impair medication adherence or BP control b. Review the patient's electronic medical records and perform a structured medication history of a medication and herbal therapies prescription and nonprescription c. Reinforce strategies to maintain BP control and provide adherence aids as necessary d. Work closely with the primary care physician to create a long-term care plan e. Present recommendations to patients' primary care providers II. Timeline and payment information a. Contract will run for 12 months: Sept 30, 2010 to Sept 30, 2011 i. Task a = $__________ ii.
