- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. W81K00-10-T-0372 is issued as a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. This acquisition is unrestricted under NAICS Code 339114, Size Standard 500 employees. The requirement in this solicitation is: one E4D Dentist Chairside CAD/CAM System to include wand milling machine, job server, computer, keyboard, wireless mouse, manipulation software, 3 watt Vivident laser, three year warranty, three year quarterly preventative maintenance, three year on-site support, three year software upgrades, installation, on-site training, off-site training for two individuals at manufacturer location, two each operators manuals, and two each service manuals. Manufacturers: D4D Technologies, LLC and Vivident. Quantity: 1 system. Vendors offering other than the brand name product shall meet or exceed the following Salient Characteristics and provide product literature, manufacturer, product included with system, and product numbers with quote: CAD/CAM System must be able to, or include: Produce single unit, full contour crowns, inlays, onlays, and veneers; allow for same day restorations; 3D design software and 3D scanning analysis; in office milling; intra-oral digitizer laser scanning device; portable design center with wand milling machine, job server, computer, keyboard, and wireless mouse; installation; on-Site Training, Fort Hood, Texas; training at manufacturer location for two individuals; two each operator and maintenance manuals; starter material for system operation; three year warranty on all hardware; three year, quarterly, preventative maintenance by trained service technicians; three years of software upgrades; and three year access to remote and live support. Delivery date is no later than October 31, 2010. Please provide your delivery date, on receipt of order, with quote. Place of delivery and acceptance is Carl R.
