Solicitation Number: Notice Type: VA-258-10-RQ-0336Combined Synopsis/Solicitation Synopsis: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. Reference attached Solicitation VA-258-10-RQ-0336 being issued as a Request for Quotation. The attached solicitation (SF 1449) incorporates provisions and clauses for commercial items. All quotes shall be guaranteed until 30 Sept 2010. Attached is the solicitation VA-258-10-RQ-0336. In addition, this is a brand name specification for Edwards Lifesciences manufactured products only, IAW FAR 6.302-1. Only one responsible source and no other supplies or services can meet the agencies needs. These items are Vigileo monitors that allow for the continuous monitoring of essential hemodynamic patient information. The hospital currently owns 3. The same brand monitors must be used to retrieve and maintain consistent hemodynamic patient information from one unit to the other. Currently the VA Supply Processing and Distribution (SPD) department purchase compatible Flotrac sensors and PreSep catheters for this monitor. If the same monitor is not purchased, SPD will have to purchase and stock two different types of sensors and catheters, with a risk of confusion in SPD personnel and nursing on which ones correspond to what monitor, in turn endangering the lives of our patients. This acquisition is reserved for a Service Disabled Veteran Owned Small Business concern. The following provisions in their latest editions apply to this solicitation: FAR 52.212-1, Instructions to Offerors, Commercial, FAR 52.212-2, Evaluation of Commercial Items. The Government will award a firm-fixed price contract, resulting from this solicitation, to the responsible offeror, whose offer conforming to the solicitation will be the Best Value to the Government. The Best Value is the most advantageous offer, price and other factors considered, and consistent with the Governments stated importance of evaluation criteria. The following factors shall be used to evaluate offers: Price and delivery schedule. Therefore all offerors must submit specifications of the Vigileo monitor, their best price and best delivery schedule. The government intends to evaluate quotes and make award without discussions with offerors (except clarifications as described in FAR 15.306(a)). Therefore, the offerors initial quote should contain the offerors best terms from a cost or price standpoint. Do not assume you will have the opportunity to clarify, discuss or revise your offer. Offeror may, at the discretion of the Government, be asked to provide more information and clarification regarding their offer/quote. Request for such information does not constitute discussions. The Government reserves the right to conduct discussions if the Contracting Officer later determines them to be necessary. FAR 52.212-3 (Alt 1), Offerors Representations and Certifications, Commercial Items (offerors shall include a completed copy as part of their quote).
