- STATEMENT OF WORK (SOW) DEPARTMENT OF VETERANS AFFAIRS - WILMINGTON VA MEDICAL CENTER SUPPLIES OR SERVICES Contractor to provide Radiopharmaceutical products as listed herein, as requested by authorized VA Staff. A.UNIT DOSE ITEMS: These items are to be provided on a unit dose bases, or otherwise specified, pre drawn into a syringe, calibrated and delivered to the VA on Date and time specified when the ordered is placed. DESCRIPTION/SPECIFICATIONS 1.Contractor shall furnish and deliver radiopharmaceutical supplies as LISTED HEREIN TO THE Wilmington VA medical Center, Wilmington, Delaware. 2.Contractor will provide for the placement of one or more telephone delivery order (s) for items listed herein 24 hours per day, seven days a week including holidays. 3.Contractor must be licensed by the Nuclear Regulatory Commission and be regularly established in the business of providing radiopharmaceuticals. Offeror must provide copies of licenses and certifications along with offeror to the Contracting Officer. 4.Contractor shall provide on an as-needed basis miscellaneous supplies and/or services including but not limited to the following: a)OSHA compliant monitoring devices. b)Sealed sources for QA procedures. c)Dose Calibrator calibration. 5.DELIVERY ORDERS: Normally, orders shall be placed during the workday and delivery shall be required by 7:00 am the following workday. Due to the demands of the Medical Center patient care requirements, there shall be no restriction to the number of orders which may be placed by the VA, nor the number of deliveries which may be required per day. In the event that the requirements may be determined in advance, a longer delivery time will be scheduled accordingly. The delivery order shall specify: contract item(s) and quantity, time(s) of delivery and applicable VA order number. Orders can be placed orally by telephone and/or fax transmission. Contractor shall routinely provide two (2) deliveries per day. Any additional deliveries will be subject to the emergency delivery fee. 6.DELIVERY LOCATION: Deliveries will be made to: VAMC WILMINGTON, DELAWARE: Wilmington VA Medical Center Attn: Nuclear Medicine Department Hot lab, Room 1601 Kirkwood Highway Wilmington, Delaware 19805 Access to this room during non-business hours shall be obtained by contracting VA Police Service or a key shall be provided by the Contracting Officer's Technical Representative (COTR). 7.EMERGENCY ORDERING PROCEDURES: An added cost is provided for additional deliveries in excess of two (2) per day. Depending upon the availability of the product, the Contractor shall make deliveries as specified when orders are placed between the hours of 7:30 am and 3:30 pm and within 2 ½ hours when orders are placed between the hours of 5:00 pm and 7:30 am. 8.DELIVERY COMMITMENT: Time of delivery specified or mutually agreed to at the time of receipt of telephone order shall become mandatory upon the Contractor. Failure to perform in accordance with the delivery commitment may be grounds for termination of contract I accordance with the provisions for default. 9.PERSON'S AUTHORIZED TO PLACE ORDERS: VAMC WILMINGTON, DE: The following VAMC Staff are authorized to place orders under this contract: Chief Nuclear Medicine Service Supervisor, Diagnostic Imaging Service Nuclear medicine Technologist 10.PROCESS OF DETERMINING CONTRACT AWARD: Award shall be made contingent upon Best Value and the offeror's ability to meet or exceed ALL requirements and evaluation factors. 11.SOLICITATION: The solicitation adheres to the format defined in FAR 12.6 and FAR 13.5 and VAAR Part 873 for the acquisition of commercial products. 12. DEFINITIONS: a)Contracting Officer (CO) - The person executing this contract on behalf of Government with the authority to enter into and administer contracts and make related modification, determinations and finding. b)Contracting officer's Technical Representative (COTR) - A person Who takes necessary action to insure the contractor performs in accordance with and adheres to the specifications contained in the contract, and to protect the interest of the Government. PAST PERFORMANCE EVALUATIONS: Prospective bidders/offerors are hereby notified that past performance evaluations will be conducted using information obtained from the Contractor Performancae Suystem (CPS). Past Performance Information Retrieval System (PPIRS), and / or any other sources deemed appropriate. 15.NATIONAL HOLIDAYS: Services shall be available upon request twenty-four (24) hours a day, three hundred sixty-five (365) days per year, including all National Holidays.
