- The Department of Veterans Affairs Medical Center 100 Emancipation Drive Hampton, VA 23667 is seeking potential sources capable of providing limited dental services for eligible Veterans who are enrolled in the VA Homeless Program. The dental facility must have the following unique and beneficial qualifications; access close to Hampton VAMC where most of these entitled Veterans reside but do not have vehicles/transportation; must be wheelchair accessible and have reasonable access to public transportation or utilization of VA shuttles for the Veterans to get to and from that facility. The homeless population requiring these services fluctuates and will vary. It is estimated that approximately 15 patients per month will be referred for evaluation and treatment. The goal of the dental treatment will be to alleviate pain, eliminate foci of infection and replace teeth that will increase functionality or esthetics. This is the basic treatment that can restore a patient ™s oral health. The scope of care includes exam with appropriate X-rays, oral hygiene instruction with nutritional and tobacco counseling, oral prophylaxis, scaling and root planning, extractions with local anesthesia and necessary oral surgery to the ridge for prosthesis, restorations in amalgams or tooth colored composite resin, endodontics without crown, resin based partial or complete dentures. The applicable NAICS Code is 621210 with a size standard of $7.0 Million. No formal solicitation document exists at this time. The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. Responses to this sources sought notice should include the following: name and address of company; business size (large or small); identify whether or not firm falls into any of the following socioeconomic categories “ 8(a), HUBZone, Service Disabled Veteran-Owned; brief description of qualifications and any other pertinent information, which demonstrates ability to meet the above requirement. Capability statement that addresses the organizations qualifications and ability to perform the work described above. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition.
