- This is a COMBINED SYNOPSIS/SOLICITATION for Commercial Items prepared in Accordance with the Information in FAR Subpart 12.6, using Simplified Acquisitions Procedures for commercial items found at FAR 13, as supplemented with the addititional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will not be issued PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. The Request for Quotation (RFQ) number is N00259-10-T-0693 The closing date is SEPTEMBER 23,2010 @ 3:00 p.m. Pacific Standard Time. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-44. It is the contractor ™s responsibility to be familiar with the applicable clauses and provisions. Naval Medical Center San Diego requests responses from qualified sources capable of Providing Refurbishing program for instruments and instrument set used in main operating room during surgical procedures. CLIN 0001 Service (1) Establish contract for refurbishing program for instruments and instrument set used in main operating room during surgical procedures. Sets must be refurbishing (2) Times a year. Period of Performance Oct. 1, 2010 thru Sept. 30, 2011. The following factors to use to evaluate offers. (1) Technical Capability- Defined as an assessment of the prospective contractor ™s Key management personnel to determine if they have the basic knowledge, experience, and understanding of the requirements necessary to provide the required services ¯. (2) Past Performance- provide three references addressing that you have provided the same or similar services in the past 3 years. (3) Price- Technical capability and past performance, when combined are significant evaluation factors that will be used to arrive at a best value award decision. NOTE: Contractors who do not meet the first 2 factors will not undergo price evaluation. The award will be made on all or none basis. Offeror must complete and submit with the quotation FAR provision 52.212-3 Representations and Certifications-Commercial Items ( May 2008).
