Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will not be issued. Solicitation No. RMN00100353 herein cited will be procured under FAR Part 12, Acquisition of Commercial Items. Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-43. This acquisition is a total small business set-aside. NAICS Code: 611710, Small Business Size Standard: $7.0 million. DESCRIPTION: Math and Data Consulting Services for BIE, Ojo Encino Day School, Cuba, NM STATEMENT OF WORK Mathematics and Data Consulting Servicesfor BIE, Ojo Encino Day School, Cuba, NM Background:In order for Ojo Encino Day School to continue to make progress towards meeting their school goal of improving mathematics learning and teaching for all grade levels, K-B, they will need continue support in the areas of both mathematics and informative data analysis. Progress has been made during the last three years in terms of lesson planning, curriculum implementation, and the use of data-both classroom data and standardized test data-to inform classroom and school decision making in the area of mathematics. The purpose of this proposal is to supply on-going instructional support in mathematics and data analysis (in all assessed subject areas) for the school. Scope:The school will receive on-going professional services by a professional in mathematics education to support its instructional program and the school's efforts to achieve Adequate Yearly Progress. In addition to instructional support and professional development in mathematics, the consultant would also work with the school staff to analyze the various assessment tools such as the NWEA, NMSBA, and DIBELS used by the school to make data-driven decisions. Objectives:Provide on-site technical assistance and professional development to include:olesson plan development with teachers based on the performance and assessment objectives for students in mathematics in the state of New Mexico and fidelity to the instructional program adopted by the school;observation of mathematics instruction in K-8 classrooms to assess overall implementation of lesson plans and fidelity of textbook and to provide feedback and recommendations for improvement;modeling of best practices in mathematics for teachers and staff;the provision of in-service on specific strategies for teaching mathematics such as making the process standards an integral part of instruction and boosting problem - solving skills;provision of in-service related to progress monitoring on assessments currently in place at Ojo Encino (Dibels, NWEA, NMSBA, formative teacher assessments from their textbook);provide support and guidance to the school's leadership team related to effective implementation of school-wide mathematics programs. Tasks:The provider will be on site for 45 days during the 2010/2011 school year(as determined by the principal/acting principal) and perform the following tasks:a)Observe mathematics instruction in all K-8 classrooms and debrief all observationsb)Model effective mathematics instruction with K-8 teachers focusing on cohesive lesson planning and direct instructional modelsc)Provide assistant and training in curriculum/lesson mappingd)Provide and assist teachers with diagnostic analysis of student work on unit exams, pre-tests, and other classroom worke)Provide in-service on mathematics content area as determined by the staff and administration (for example, data and probability, teaching problem solving strategies, integrating writing into mathematics classes,...)f)Review school data and conduct appropriate statistical analyses to determine student/school growth and areas of greatest need and use those results to help individual teachers monitor and adjust their curriculum maps based on their classroom/school data. Timeline:The consultant will provide services beginning from date of contract through May 20, 2011. Service provisions will be scheduled with Ojo Encino based on their articulated needs. Suggested dates for the 45 days are given below. Ojo Encino has the final approval on scheduling. September 27-October 8, 2010: 10 days October 18-22, 2010: 5 daysOctober 25-29, 2010: 5 daysNovember 15-19, 2010: 5 daysJanuary 24-28, 2011: 5 daysFebruary 14-February 18, 2011: 5 daysMarch 7-18, 2011: 10 days PROPOSAL LINE ITEMS: 1. Consulting Services for Ojo Encino Day School, 45 days @ $_______________ per day = $_______________. 2. Mileage Cost $ ____________ 3. Travel Cost: $_______________Travel cost incurred for transportation, lodging, meals, and incidental expenses (as defined in the regulation cited in FAR 31.205-46) shall be considered to be reasonable and allowable only to the extent that they do not exceed on a daily basis the maximum per diem rates in effect at the time of travel as set forth in the Federal Travel Regulation. The consultant will be reimbursed for travel to provide support at a Government site or other site as may be specified and approved by the COTR under this effort. All travel shall be approved, by the COTR, prior to commencement of travel. Consultant shall provide supporting documentation and a detailed breakdown of incurred travel costs with each invoice. BACKGROUND INVESTIGATION: In accordance with 25 U.S.C. 3201 (P.L. 101-630), Indian Child Protection and Family Violence Prevention Act; 42 U.S.C. 13041, Crime Control Act of 1990; 441 Departmental Manual, Personnel Security and Suitability Requirements all positions with duties and responsibilities involving regular contact with or control over Indian children are subject to a background investigation and determination of suitability for employment. Accordingly, all such positions require a favorable screening (applicant and suitability) determination made by the Bureau of Indian Affairs (BIA) Office of Indian Education Programs (OIEP), Personnel Security and Suitability Program; and a favorable background investigation determination conducted by the Office of Personnel Management (OPM). A favorable screening (applicant and suitability) determination must be made by the Bureau of Indian Affairs (BIA) Office of the Indian Education Programs (OIEP), Personnel Security and Suitability Program prior to contract award or made within 60 days of incorporation of this clause into a current/existing contract. Pending completion of a background investigation by OPM, the Contractor who has been granted a favorable screening (applicant and suitability) determination must be within the sight and under the supervision of a Government employee who has had a completed background investigation (42 U.S.C 13041 (3)) during contract performance until a final background investigation determination has been made. Prior employment with the Federal Government, including BIA, OIEP or the Bureau of Indian Affairs, does not assure that the appropriate screening or investigation was conducted nor does it alleviate or guarantee a favorable screening (applicant and suitability) determination or background investigation. Prior to an award or during the 60 day period discussed in paragraph two, it is solely the company, individual or contractor's responsibility to contact the BIA, OIEP Personnel Security and Suitability Program at 505-563-5287/5288/5289/5294 in order to obtain a complete security package, which includes but is not limited to appropriate investigative forms and/or inquire about their previous investigation; and, obtain a screening (applicant and suitability) determination, and subsequent final background investigation. If an unfavorable screening (applicant and suitability) determination is made, a purchase order shall not be awarded to the individual who received an unfavorable screening or to the company who has proposed to employ the individual to perform the work. The individual and or the company who proposed to employ this individual may be determined to be non-responsible if an unfavorable screening (applicant and suitability) determinations is made, which will make them ineligible for purchase order award. If an unfavorable final background investigation determination is made, the purchase order may be terminated and the employee in question shall not be authorized to provide service to the BIA, OIEP, if the services involve contact with or regular control over Indian children. Award will be made to the lowest priced offeror, whose quotation is conforming to the requirements herein, will be most advantageous to the Government, and is fair and reasonable.
