Description
This is a Brand Name only requirement combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and the incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-44. The Veterans Health Administration at Seattle, Washington is soliciting bids for a contractor to provide the following items: Contract Line Item Number CLIN 0001: 3 EA - AutoCat 2 Wave Intra Aortic Balloon Pump System; item # IAP-500 CLIN 0002: 3 EA - Autocat 2 Wave Accessory Bag; item # IAA-5003. CLIN 0003: 3 EA - Dual Hangar IV Pole; item # IAA-00175. CLIN 0004: 1 EA - Electronic Simulator; item # IAT-00201. CLIN 0005: 3 EA - Clin Five (5) is for a request for Trade-in pricing the contractor will offer to the VA on their quote for the three used IABP's, we are replacing. These IABP systems are approximately 10 years old. Delivery of all items is desired to be no later than 20 days after receipt of order, FOB Destination. This is a Brand Name only purchase. The Contracting Officer is not responsible for locating or obtaining any information not identified in the quote. The selection resulting from this request for quotations will be made on the basis of the lowest-priced quote from a responsible quoter with acceptable or neutral past performance. This solicitation will result in a firm fixed-price contract. The Request for Quote (RFQ) number is VA-260-10-RQ-. The applicable NAICS code is 339112. Offers are due no later than 23 September 2010 at 3:00:00 PM Eastern Standard Time. The provision at FAR 52.212-1, Instructions to Offerors Commercial (JAN 2004), applies to this acquisition. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items (OCT 2003) applies to this acquisition. FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders --Commercial Items (Jul 2010) applies to this acquisition. All offers must include a completed copy of this provision. All offers must either be accompanied by Representations and Certifications or the offeror must be currently registered in ORCA at website https://orca.bpn.gov/.
