PERFORMANCE WORK STATEMENT (PWS) FOR Hyperbaric Maintenance Technician-Apprentice 19 June 2010 Section One 1. Description of Services. 1.1. Requirements. The Contractor shall provide non-personal services for Hyperbaric Maintenance Technician referred to as contract personnel at Military Medical Treatment Facilities in San Antonio, Texas. Locations may include, but are not limited to, Wilford Hall Medical Center (WHMC) at Lackland AFB and Building 160W Brooks-City Base, San Antonio, Texas. WHMC may also be referred to as the Medical Treatment Facility (MTF), 59TH Medical Group, Clinics, annexes, and any successor organization(s) of WHMC, if applicable. Contract personnel shall perform services and support compatible with the medical facility's operating capacity and equipment. 1.1.1. Specific Tasks. The contract personnel tasks shall include, but are not limited to: Hyperbaric Maintenance Technician directly performs maintenance and safety inspections of over $4M hyperbaric operations and assists with long-range planning and design of crucial hyperbaric chamber equipment, retrofits chamber for modernizing equipment; performs critical new upgrades on aging chamber equipment ensuring optimum patient care treatment. Operates, maintains, and inspects six complex hyperbaric chambers and related support equipment valued at over $6M. Monitors patients as the Chamber Operator or Crew Chief. Assists in patient treatment process and disposition. Coordinates with medical and maintenance personnel on chamber equipment configuration and modification for patient hyperbaric treatment and hyperbaric medicine research. 1.1.2. Position Responsibilities. Responsibilities of the contract technician are as follows: periodic inspection of compressors, maintain library of hyperbaric branches' equipment manuals, may perform duties as PMEL coordinator and/or monitor, and perform plumbing in hyperbaric maintenance as required. Maintains, repairs, and inspects Class A, B, and C hyperbaric chambers. Maintains, repairs, and inspects hyperbaric chamber fire suppression systems. Maintains, repairs, and inspects the setup, operation and safety of Class A, B, and C chambers. Maintains, repairs/inspects intercom systems, patient entertainment system, television monitors, cameras and (digital) video recording equipment. Installs, maintains, repairs and inspects new medical and non-medical equipment. Maintains, repairs, and inspects wheelchairs and gurneys for inside chamber use. Designs and builds equipment for use in hyperbaric chambers. Performs Chamber Crew Chief and Chamber Operator duties. Performs general maintenance for the entire hyperbaric facility; and conducts research and submits requests to equipment/supply custodians for new and replacement equipment. Maintains bench stock item inventory. 1.2. General Information. 1.2.1. Contractor Representative. The contractor shall identify the name and local telephone number of the contractor's local point of contact to the Contracting Office (CO) in writing prior to beginning performance. The contractor shall identify in the designation letter any limitations on it's representative's authority to act on behalf of the contractor. The contractor shall provide an updated designation letter whenever any changes occur. The contractor representative may be required to meet with the government during the performance of this contract at the request of the CO. 1.2.1.1. The Contractor Representative shall respond to a request to meet with the CO within 24 hours of notification under routine circumstances. Crisis situations shall be handled on a case by case basis as circumstances dictate. 1.2.2. Hours of Performance. Hyperbaric clinics require coverage during normal daily hours of 0655 to 1600, with the following exceptions: chamber maintenance personnel will pull a 7 day call schedule for after hour's emergent hyperbaric treatments. Personnel working in these areas may be scheduled for any shift to include rotating shifts and weekends. The duty day consists of eight (8) hours not including the lunch break. The duty week shall not exceed forty (40) hours in seven (7) consecutive calendar days. The Contractor shall provide full coverage (continuous presence) during the required hours of performance. The Contractor shall maintain personnel continuity to the maximum extent consistent with efficient resource management. 1.2.3. Staffing. The Contractor shall provide qualified and trained contract personnel to cover the required hours of performance. The Contractor shall staff and contract personnel to meet the requirements in this PWS and ensure continuity of performance. If the contract personnel are absent for more than three (3) consecutive working days, the Contractor shall initiate replacement action on the fourth day. 1.2.4. Scheduled Absences. Scheduled absences shall be scheduled thirty (30) calendar days in advance and mutually agreed upon by the quality assurance (QA) monitor or government designated representative, and the contractor. 1.2.5. Unscheduled Absences. Unscheduled absences shall be reported to the contractor or Program Manager no later than two (2) hours prior to the start of shift by the contract personnel communicated to the QA monitor or government designated representative 1.2.6. Public Holidays. Federal law establishes the following public holidays: New Year's Day (Jan 1); Birthday of Martin Luther King, Jr. (third Monday in Jan); Washington's Birthday (third Monday in Feb); Memorial Day (last Monday in May); Independence Day (Jul 4); Labor Day (first Monday in Sep); Columbus Day (second Monday in Oct); Veterans Day (Nov 11); Thanksgiving Day (fourth Thursday in Nov); and Christmas Day (Dec 25). 1.2.6.1. Family/Down Days. Typically, Air Force Materiel Command (AFMC) designates an "AFMC Family Day" in conjunction with the following public holidays: Washington's Birthday, Memorial Day, Labor Day, and Thanksgiving Day. Specific Family/Down Days are dependent on major command directives, facility location, mission requirements, and installation commander directives. Commanders/Directors may implement reduced manning postures on these designated days. The Contractor shall not bill the Government for the Family/Down Day unless the contract personnel are required to report to work. 1.2.7. Inclement Weather. In the event of inclement weather (snow, ice, etc.), the government will notify the contractor whether its personnel shall report and, if so, when. The contractor will instruct its personnel scheduled to report for duty (who have not yet begun their shift) whether to report and, if so, when. If the government directs the contractor not to report for any shift, the government will not pay the contractor for that shift or those shifts. 1.2.7.1. The contractor's personnel on duty at the onset of inclement weather (as declared by local base officials) shall remain on duty until the end of their scheduled shift unless otherwise directed by their employer based on government direction to the contractor. 1.2.7.2 In the event that the contractor's personnel are on duty and cannot leave the installation due to inclement weather (as declared by local base authorities), the contractor will be allowed to invoice for overtime hours only if the government determines the contractor's personnel are necessary for MTF operation and instructs the contractor accordingly. 1.2.8. Closures. During Command declared closure of the facility the contractor personnel shall be treated in the same manner as DoD non-essential civilian employees. 1.3. Qualifications. The Contract Personnel shall meet the following qualification requirements. 1.3.1. The technician will have had a minimum of four years of progressively responsible experience as an instrument repairman, chamber maintenance technician rating on a year for year basis. 1.3.2. The technician will also have experience in high-pressure lines and compressors. 1.3.3. The technician will possess and demonstrate soldering skills equal to those requirements of high quality. 1.3.4. The technician will have the ability to install, maintain, repair, overhaul, troubleshoot, modify, conduct and test electronic systems. 1.3.5. The technician will be able to solve complex problems by interpreting manufacturer's manuals or similar documents with minimal technical guidance as required, from a supervisor or higher level technician. 1.3.6. The technician will be able to assist in developing training requirements and to provide training as needed. 1.3.7. The technician is required to have a working knowledge of computer systems to include operating systems, communications associated peripherals, and one programming language. Technicians will also be required to enter chamber dive data into the Hyperbaric Medicine Program or equivalent program. 1.3.8. The technician will have an operational understanding of how all of the above systems function independently and as a single system, and how they are used to test and evaluate patient environments and AF operational system environments. 1.3.9. The technician will be a graduate of a forty (40) hour Undersea and Hyperbaric Medical Society (UHMS) Designated Introductory Course in Hyperbaric Medicine or the USAF Clinical Hyperbaric Technician Course. They shall be knowledgeable about compression physics and the mechanical effects of pressure change as it relates to the hyperbaric chamber and its occupants. The technician shall understand the physiological basis and clinical aspects of decompression sickness and arterial gas embolism. The technician will understand and use U.S. Navy decompression tables, NOBENDEM decompression tables, and hyperbaric patient treatment tables as directed. The technician shall set up and operate specialized medical equipment. Additionally, the candidate shall apply chamber safety to the everyday working environment and follow emergency procedure checklists in the event an emergency occurs. 1.3.10. Clinical Hyperbaric Technician (CHT) certification from the National Board of Diving and Hyperbaric Medical Technology is desirable but not mandatory for hire. However, the contractor shall ensure the technician acquires CHT certification within eighteen (18) months of hire date. 1.3.11. The contractor should have experience handling blood and body fluids and other potentially infectious material soiled patient linens. Associated Professionals in Infection Control and Epidemiology (APIC) infection control safety standards shall be used at all times. 1.3.12. Contractor shall be able to lift 40 lbs. 1.4. Education. The contract personnel shall meet education requirements. 1.4.1. Continuing Education Units (CEU). Required continuing education for CME shall be at no additional cost to the Government. The contract personnel shall provide appropriate CME documentation to the QA monitor or designated government representatives within one month after all CME accredited events. 1.4.2. The contract personnel shall have and maintain a current Basic Life Support (BLS) certification with the American Heart Association or Red Cross agencies. 1.4.3. The contract personnel shall complete a UHMS approved Hyperbaric Safety Director Training course NLT between the 12th and 18th month of employment. This course meets CME requirements and contributes to CHT certification. 1.4.4. The contract personnel shall complete the Initial profile package requirements. Refer to Attachment 1. 1.5. Compliance. The contract personnel shall comply with all applicable Air Force Instructions (AFI), Department of Defense (DoD) Regulations, and Medical Wing Instructions (MDWI) as outlined in Attachment 2. 1.6. Documentation. The contract personnel shall prepare and manage all data created for Government use or legally controlled by the Government, all documentation to meet or exceed established standards as set forth in the most current version of Air Force Instruction 41-210 Patient Administration Functions and the Air Force Records Disposition Schedule (AF RDS) located at https://afrims.amc.af.mil/rds_series.cfm or http://www.amc.af.mil/shared/media/document/AFD-060803-003.pdf. 1.6.1. Conduct Requirements. The government reserves the right to restrict the performance on this contract by any individual who is identified as a potential threat to the health, safety, security, general well-being or operational mission of the MTF and its population. 1.6.2. The contract personnel shall not advise, recommend or suggest to persons eligible to receive medical care at Government expense that such person should receive care at an outside agency or provider at any place other than as designated under this contract. 1.6.3. The contract personnel shall not use Government facilities or other Government property for personal or other business not related to this contract. 1.6.4. The contract personnel shall not respond to any media inquiries nor provide interviews, comments, or any other responses to the media. All inquiries or complaints from the media or other sources shall be immediately relayed to the QA monitor or designated government representatives. 1.7. Security. 1.7.1. Contractor Notification Responsibilities. The Contractor shall notify the 37 SFS Industrial Security Office at each operating location prior to the performance of the contract. The notification shall include the contract number, contracting agency, the date service performance begins and ends, location(s) of service performance and future performance, and the reason for the contract (i.e., work to be performed). 1.7.2. Obtaining Pass and Identification Items. The Contractor shall ensure that the pass and identification items required for contract performance are obtained for the contract personnel providing service. The Government will provide CAC and MTF ID badges which will be displayed while performing tasks in the MTF. 1.7.3. Investigation Requirements. The contractor providing service under this contract is required to initiate the Electronic Personnel Security Questionnaire (EPSQ), 59th MDW/LS Contracting Services will send a CD and instructions to complete the EPSQ when a contract is awarded. SF85P Questionnaire for Public Trust Positions is to be completed and printed and electronic copies (CD) are to be hand carried to the Unit Security Manager prior to performance. The contractor shall have proof of U.S. citizenship (certified copy of birth certificate or naturalization papers) or a valid U.S. Immigration Form 1-151 and Alien Registration Card ("Green Card"). As a minimum, the contractor providing service requiring operating government-furnished workstations that have access to Air Force e-mail and requiring unescorted entry to a restricted area/Controlled Areas, shall successfully complete a National Agency Check (NAC). The contractor shall coordinate and schedule appointments with the MTF Security Manager prior to performance. The unit security manager will initiate the AF FM 2583 Request for Personnel Security Action, review the security questionnaire and request a Local Background Check from 37 SFAR prior to scheduling an appointment with the 37 Security Forces to submit the EPSQ. This security questionnaire will be completed/ processed by the 37 SFS Personnel Security Office prior to contract performance. The contractor is responsible for monitoring the coordination and completion of the scheduled appointment date and time with Security Forces. The security questionnaire reviewed by the unit security manager will be hand carried by the contract personnel to the 37 SFS Personnel Security Office for processing prior to contract performance. Upon completion, the contract personnel shall return the AF FM 2583 to the unit security manager. 1.7.4. Listing of Contract Personnel. The contractor shall provide the CO and 37 SFS/SFAD a list of contract personnel within five (5) calendar days after receiving award or Notice of Award. The list shall include the name, social security number, and date of NAC investigation if contract work involves unescorted entry to Air Force-restricted or other sensitive areas designated by the installation commander. The list shall be validated and signed by the Contractor Representative. The Contractor shall maintain this listing throughout the tenure of this contract. Updated listings shall be provided when contract personnel status or information changes. 1.7.5. Physical Security. Areas accessed by the contract personnel shall comply with base instructions for FPCON procedures, Random Antiterrorism Measures and local search/identification requirements. 1.7.6. Keys and Lock Combinations. The contract personnel shall abide by all current MTF key control and lock combination procedures. The Contractor shall be responsible for any keys issued to the contract personnel. The keys shall not be duplicated. The Contractor shall ensure its contract personnel do not allow government-issued keys to be used by personnel other than those currently authorized. No unauthorized personnel shall be allowed entry into the locked area. Keys shall not be used to open work areas for personnel other than personnel engaged in performance of their tasks, unless authorized by the Government functional area chief. The contract personnel shall use keys issued to them to secure/open areas only as directed by the QA monitor and/or functional area chief. Lost keys and/or locks shall be reported to the issuing party and QA monitor immediately upon recognition of the loss. The QA monitor shall notify the CO of the lost key. The contract personnel shall be responsible for the replacement costs of any lost keys and any associated locks. Reimbursement shall be provided before the Government replaces lost keys or performs re-keying. 1.7.7. Internal Operating Instructions. The Contractor/contract personnel shall abide by host unit's current Operating Instructions (OIs) for internal circulation control, protection of resources, and regulated entry into Air Force controlled areas during normal, simulated, and actual emergency operations. 1.8. Removal of Contractor Personnel. The government reserves the right to require immediate temporary or permanent removal of any individual whose actions raise reasonable suspicion that patient care or services may be compromised in any way or that pose a threat of harm to other contractor/government personnel or self. The contract personnel may be required to submit to random drug/alcohol testing. The contract personnel who refuse testing shall be permanently removed from contract performance. Removal of any individual does not relieve the contractor of any performance requirement. 1.8.1. If a removal occurs, the QA monitor will contact the CO and the contractor's representative within twenty-four (24) hours. A meeting may be required with the CO, QA monitor and Contractor Representative to discuss further action. Notification of all removals will be made by the CO. 1.8.2. The CO will notify the Contractor if and when permanent removal is required. In the event of disagreements between the Government and the Contractor, the decision of the CO will be final. During the period of time between the removal and the final decision of the CO, the Contractor agrees to provide backup/replacement contract personnel in accordance with the terms of this contract. 1.9. Health Requirements and Immunizations. All Contractor personnel shall control and prevent disease in accordance with AFI 48-105 Surveillance Prevention and Control of Diseases and Conditions of Public Health or Military Significance, and the American Public Health Association Publication, Control of Communicable Diseases Manual, and the Centers for Disease Control and Prevention (CDC) publication, Morbidity and Mortality Weekly Report, Air Force Policy (as applicable) and its supplements. The Contractor shall provide proof of immunization or immunity from or screening for the following diseases according to Air Force guidelines equivalent to that provided to active duty armed forces: Hepatitis B, MMR (Measles, Mumps, and Rubella), Varicella, Tdap (Tetanus, Diphtheria, and Pertussis), Tuberculosis (TB), HIV (contract personnel who have direct patient care) and influenza (IAW 1.2.8.2.) for all personnel prior to contract performance. For the TB screening, the Contractor shall provide proof of negative TB 2-step skin testing IAW CDC recommendations outlined in attachment one (1) of this document. If an individual has a past positive TB skin test, the Contractor shall provide proof of a negative chest X-ray within twelve (12) months) prior to personnel performing. The Government reserves the right to require more frequent Tuberculosis testing of personnel based on the facilities' annual TB risk assessment. Additional vaccines as they are made mandatory for health care workers may also be required in unusual circumstances. 1.9.1. Medical Tests. The Contractor shall not obtain medical tests or procedures at the MTF, unless identified in Section three (3); the Contractor shall be responsible for the cost of any emergency medical services received. 1.9.2. Influenza Immunization. Contractor Personnel shall receive the current influenza immunization, at contractor's expense, equivalent to that provided to active duty armed forces personnel unless contraindicated by allergy; as an example, difficulty breathing, itching, and other symptoms of anaphylactic hypersensitivity and/or physician's diagnosis. At Government's discretion it can be accomplished as stated in paragraph 1.9.2.1 of the PWS. 1.9.2.1. Influenza Vaccine. Vaccine will be provided by the Government, if available as determined by the MTF. Although this vaccine may be provided by the Government, it may be obtained at other facilities with the cost being borne by the Contractor. Unless vaccinated by the Government, proof of vaccination will be required. 1.9.3. Emergency Healthcare. The MTF will provide emergency healthcare for injuries occurring while on duty. The Contractor shall reimburse the government for such services. This includes blood borne exposure protocols after start of work. The Government will provide initial testing at no cost, but the Contractor shall provide follow-up testing or reimburse the government for such services performed within the MTF. The Contractor will provide the MTF with documentation for follow-ups performed outside the MTF as required by MTF policy. The Government will provide post-exposure tuberculosis testing at no charge to the Contractor when required (ex. following exposure to active TB case with in MTF) to avoid testing delays. 1.10. Initial, Quarterly and Annual Reports. The Contractor shall submit initial, quarterly, and annual reports to the QA monitor and/or credentials office (as applicable) for use in monitoring performance. Such reports may include, but are not limited to: 1.10.1. Health and Immunization requirements (initially and annually) 1.10.2. Personnel Changes (quarterly) 1.10.3. Basic Life Support (BLS) (initially and renewal)*. As accepted by American Heart Association (AHA) or American Red Cross 1.10.4. Competency and currency of licensure/registration (initially and renewal) 1.10.5. Security (initially renew per AFI) 1.10.6. Timesheets (monthly) in WAWF and signed by QA monitor or Government Representative. *Note: In accordance with Military Training Bulletin NO. 07-003 (7 Feb 07), this training will not be provide by the government. Section Two. 2. Service Summary. P.0. # Performance Objective PWS Paragraph Performance Threshold 1. The contractor shall provide documentation to show compliance with educational requirements. 1.4. 100% compliance. 2. The contractor shall ensure that contract personnel comply with the responsibilities outlined in: Specific Tasks and Position Responsibilities. 1.1.1. 1.1.2. Not more than one (1) substantiated complaint per month. 3. The contract personnel or contractor will provide the COR documentation of compliance with Hours of Performance 1.2.2. 95% compliance with staffing requirements. Section Three 3. Government-Furnished Property and Services. 3.1. Facilities and Support Services. The Government will provide use of all available MTF facilities and support services, materials, publications and forms, equipment, and specialty clothing required for contract performance (except as designated in the contract). The contract personnel shall keep Government furnished supplies, equipment and work areas in a safe, orderly and clean condition. The contract personnel shall notify the QA monitor whenever maintenance of equipment is required. 3.2. Personal Protective Attire (PPA). The Government will furnish the contract personnel with appropriate common use PPA (e.g., face shield, gloves). The Government will be responsible for any repair, cleaning, and inventory required for the PPA. Items shall be used and returned as directed by the QA monitor. 3.3. MTF Orientation. The contract personnel shall attend the MTF Orientation at the outset of their working at the MTF. The MTF Orientation will familiarize the contract personnel with the policies and procedures of the MTF and will be scheduled by the QA monitor or designated government representative within two months of employment. Orientation attendance shall be required of the contract personnel during government-paid duty hours. 3.4. Training. The Government will schedule and furnish initial and annual training to include but are not limited to various applicable software, data base systems, quality assurance policies, radiation safety, local in-service, safety briefings, compliance, patient safety, performance improvement, risk management, infection control, fire protection, security, and patient sensitivity training. The classes may be scheduled individually throughout the year or scheduled at one time. 3.4.1. Security Training. The contract personnel shall be trained in accordance with AFI 31-501, AFI 31-401; AFI 33-202, Computer Security; AFI 31-101 Installation Security; AFI 33-203, Emission Security (EMSEC) and AFI 33-204, Information Assurance (IA) Protection Program. 3.4.2. Specialty Training. The Government may provide voluntary specialty training to contract personnel in a non-paid status. 3.5. Space. Any space used by contract personnel in performance of this contract may be used for other purposes during their absence. Items of clothing, personal effects, or equipment may not be able to be secured at the MTF. The Government will not incur any liability for theft, damage to, or loss of such personal items. 3.6. Telephone. The Government will provide local telephone service, Class-A telephone lines and Defense Switching Network lines limited to matters related to the performance of this contract. Personal long distance calls are not authorized. 3.7. Badges. The government will provide Common Access Card (CAC) and MTF ID badges and will be worn/displayed at all times. 3.8. Pager. When on-call, the contract personnel shall use the communication device (pager) or other electronic device issued by MTF. 3.9. Uniforms. The MTF shall furnish surgical scrubs. These items will be cleaned at the MTF's expense. 3.10. Miscellaneous Travel Cost. 3.10.1. This contract contains CLINS for miscellaneous costs. The government will reimburse authorized travel IAW the Joint Federal Travel Regulations (JFTR). 3.10.2. The funding limits specified in these CLINS may not be exceeded without prior approval from the CO. 3.10.3. Miscellaneous Costs CLINs are not subject to the application of overhead or profit. Reimbursement costs are limited to actual costs incurred. 3.10.4. Temporary Duty (TDY). Contract personnel may be required to travel to other locations for training purposes. 3.10.5. The contractor shall invoice the Government for the travel costs, along with documentation to verify the expenses. This documentation will be similar to DoD requirements for reimbursing mileage or TDY. The contractor shall furnish all receipts for items required IAW the JFTR. 3.10.6. The government shall schedule TDY requirements as far in advance as possible to allow the contractor to obtain the best travel rates. When possible, TDY should be planned at least thirty (30) days in advance of the travel.
