Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Prospective offerors are encouraged to view this site frequently for any amendment to the solicitations. This solicitation is issued as a Request for Proposal (RFP), solicitation number VA-259-10-RP-0284. The George E. Wahlen Veterans Affairs Medical Center, Salt Lake City, Utah, has a requirement for Light Fixtures. Contractor is to provide brand name or equivalent items to the line items listed. Manufacturer/brand names are mentioned not to be restrictive but to more efficiently describe the salient characteristics of the items and to offer a guide as to what is acceptable for a like or equivalent item. Any proposed œequal  products must be accompanied by a cut sheet for the product that proves it is indeed equal to the make/model included in each item description below. Proposals that do not include like or equivalent items (brand name or equal) will not be accepted and will be deemed non-responsive. STATEMENT OF WORK/LINE ITEMS: 1.Light fixture “ 4 ™x2 ™ recessed maximum security fluorescent light fixture with steel welded flange and prismatic polycarbonate lens, 120V F32T8/35K, 3 lamps, 2 ballasts equal to Morlite MAX-FDPH-24-3-T8-EB101S-4-C-F1-UNV; 32 EA. 2.Light fixture “ 4 ™x2 ™ recessed security fluorescent lay-in light fixture with one-piece housing and prismatic polycarbonate lens, F32T8/35K, 3 lamps, 2 ballasts equal to Morlite MIN-TG-24-3-T8-EB101S-3-G-F1-UNV; 85 EA. 3.Light fixture- 2 ™x2 ™ maximum security fluorescent light fixture with steel welded flange and prismatic polycarbonate lens, 120V F17T8/35K, 3 lamps, 1 ballast equal to Morlite: MAX-FDPH-22-3-T8-EB101S-4-C-F1-UNV; 11 EA. 4.Light fixture “ 1 ™x4 ™ recessed security lay-in fluorescent light fixture with one-piece housing and prismatic polycarbonate lens, 120V F32T8/35K, 2 lamps, 1 ballast equal to Morlite; MIN-TG-14-2-T8-EB101S-3-C-F1-UNV; 8 EA. 5.Light fixture “ 8  diameter security compact fluorescent downlight with decorative vandal guard polycarbonate lens, 120V F42TRT/35K, 1 lamp, 1 electronic ballast equal to Designplan; RHL-B-H5-4-O-C-C-S; 2 EA. 6.Light fixture- 12  square maximum security fluorescent light fixture with 14 gauge steel housing, flanged door and polycarbonate lens, 120V, PL13/35K, 2 lamps, 1 electronic ballast equal to Omega ORT8-1H42PLT-CS-VG-BR-IPPOLY-U; 12 EA. 7.Light fixture-48  recessed perimeter (cove) fluorescent light fixture system with frosted polycarbonate lens and step dimming, 120V, 54WT5/35K, 1 dimmable lamp, 1 electronic dimming ballast equal to Zumtobel, SLR4-PM-1545-4-U-STDM-X-X; 143 EA. 8.Light fixture “ 48  corner mount security fluorescent light fixture with steel housing and polycarbonate lens, 120V F32T8/35K, 2 lamps, 1 electronic ballast equal to Morlite MAX-CDPH89-4-2 -T8-EB101S-8-C-F1-UNV; 12 EA. 9.Light fixture- 24  vandal resistant fluorescent light fixture with extruded aluminum housing and polycarbonate lens for under-counter location, 120V F14T5/35K, 1 lamp, 1 electronic ballast equal to Luminaire VPF42HO-114-120-OP-WHT; 1 EA. 10.Light fixture “ surface (wall) mount compact fluorescent light fixture with cast aluminum housing and prismatic polycarbonate lens, 120V PL13W/35K, 2 lamps, 1 electronic ballast equal to Morlite V135-2PL13-F1-W-120; 3 EA. 11.Light fixture “ enhanced decorator full hallway LED light fixture with light sensor and clear lexan lens, 120V, equal to Pass & Seymour TMHWLECC; 11 EA. 12.Exit sign “ single face maximum security LED exit sign with battery equal to Morlite SH1206-LED-SF-CM-F1-GW-EMG-120; 4 EA. 13.Exit Sign “ double face maximum security LED exit sign with battery equal to morlite SH1206-LED-DF-CM-F1-GW-EMG-120; 6 EA. 14.Lighting control system including 4 panels equal to a Lithonia Switchpak SPAK-8S-120/277-SPAK-HVB, and 4 low voltage switches and start-up and commissioning services; 1 LT. OTHER CONTRACT REQUIREMENTS CENTRAL CONTRACTOR REGISTRATION (CCR). All contractors must register in the CCR prior to contract award. This solicitation document and incorporated clauses and provisions are those in effect through Federal Acquisitions Circular 2005-36. In accordance with 52.252-2, Clauses Incorporated by Reference, clauses incorporated by reference have the same force and effect as if they were given in full text. Prospective Offerors must also respond to 52.212-2, Evaluation of Offers. 1. Technical Capability. Contractor must be regularly established in the business called for in this solicitation. 2. Past Performance. Offeror shall provide a minimum of 3 references, including name, address, telephone number and point of contact for which the same or similar type work has been performed. The Government may, at its discretion, base past performance on past knowledge, and previous experience with the contractor, supply or service being provided, customer survey, or any other reasonable basis. 3. Price. Prospective offeror MUST provide a delivery schedule. Technical Capability and Past Performance, when combined are of equal importance when compared to price. The North American Industry Classification System code for this procurement is 423610. Contract will be awarded in the aggregate not on a line by line basis.
