- This is a combined synopsis/solicitation prepared in accordance with Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Quoters are responsible for monitoring this site for the release of any amendments or other pertinent information. This solicitation will be issued as a request for quotation (RFQ) in accordance with Federal Acquisition Regulation part 12 and 13 under the solicitation number: F3A3900233A001. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-43, Defense Federal Acquisition Regulation Supplement Change Notice (DCN) 2010-0810, and Air Force Acquisition Circular (AFAC) 2010-0402. The North American Industry Classification System (NAICS) code 238210 and the business size standard is $14,000,000.00. This solicitation is 100% set-aside for small businesses IAW FAR 52.219-6. This RFQ contains one (1) line item for the following commodity. The items are to be firm fixed priced. The items are to be in accordance with the Performance Work Statement. Refer to the Performance Work Statement for full description of this requirement. This combined synopsis/solicitation is for the following item or equal: CLIN Description Qty Unit Period of Peformance 0001 PA System for Tommy B's 1 EA Within 60 days of award Acceptance will be performed by the Government or a Government representative at designated government location. The FAR provision 52.212-1, Instructions to Offerors - Commercial Items (SEP 2006), applies to this acquisition. The FAR provision 52.212-2 Evaluation - Commercial Items (Jun 2008); Does not apply to this acquisition. Evaluation will be conducted IAW FAR 13.106. Award will be based on best value. The FAR provision 52.212-3, Offeror Representations and Certifications - Commercial Items (Jun 2008) with Alternate I. A Quoter shall complete only paragraphs (b) of this provision if the Quoter has completed the annual representations and certificates electronically at http://orca.bpn.gov. In accordance with FAR 13.004 -- Legal Effect of Quotations the following information is provided in regards to quotations: A quotation is not an offer and, consequently, cannot be accepted by the Government to form a binding contract. Therefore, issuance by the Government of an order in response to a supplier's quotation does not establish a contract. The order is an offer by the Government to the supplier to buy certain supplies or services upon specified terms and conditions. A contract is established when the supplier accepts the offer. When appropriate, the contracting officer may ask the supplier to indicate acceptance of an order by notification to the Government, preferably in writing. In other circumstances, the supplier may indicate acceptance by furnishing the supplies or services ordered or by proceeding with the work to the point where substantial performance has occurred. STATEMENT OF WORK (Tommy B's Community Activity Center) 1. Description of Work/Service: a. Scope: The contractor shall furnish all labor, materials,, tools, transportation as approved by 87FSS/FSCP, in accordance with the terms and conditions as defined in the Statement of Work (SOW). b. Location: Work is to be performed at Joint Base Tommy B's Community Activity Center, Bldg 2705 East Arnold Ave, McGuire, NJ 08641. 2. The contractor will perform the following: a. Furnish and install all equipment for Audio and Video system capabilities for the individual rooms of the community center. System must be a Multi Zone control system that can be combined into one. b. Zone 1 (Ballroom) System must include a retractable ceiling mounted screen, 1 ceiling mounted video/data projector, in ceiling mounted loudspeakers, rack mounted amplifier with digital audio processor for equalizing and controls of audio signals, DVD and CD player, both wireless hand held and label microphones, wall mounted touch panel control system with on/off system, Screen up/down, source input volume control and room wall plate for optional personal computer c. Zone 2 (Dining Room) 1 ceiling mounted video/data projector, in ceiling mounted loudspeakers, rack mounted amplifier with digital audio processor for equalizing and controls of audio signals, DVD and CD player, both wireless hand held and label microphones, wall mounted touch panel control system with on/off system, Screen up/down, source input volume control and room wall plate for optional personal computer d. Zone 3 (Starlifter Lounge) in ceiling mounted loudspeakers that would be control from zone 1 and or 2. e. Personnel. a. The contractor will hire only personnel that are qualified to do repairs and install b. The contractor must identify in writing their contract manager and/or alternate who is responsible for the performance of work to the Quality Assurance Representative (QAR). c. The contractor and all employees shall present a neat appearance and be easily recognized. Proper attire will be worn at all times. Attire will be in good taste and subject to the approval of the Director. d. The contractor is reminded that off-duty, active duty military personnel hired may be subject to permanent change of station, change of duty hours, or deployment. Their absence does not constitute a valid excuse for nonperformance under this agreement. e. Health Care: The medical treatment facility will provide urgent health care to contract employees (contract employee is not a Government employee but is a contractor or an employee of a contractor) for injuries occurring while on duty. Urgent health care is defined as medical care authorized to the extent necessary to save life or limb and prevent undue pain and suffering. The contract employee will be transferred to a civilian medical facility as soon as the contract employee is stabilized. The cost of such treatment will be paid for, in full, within a reasonable period of time, by the contract employee and will not be borne by the 87th Medical Treatment Facility, the United States Air Force or the Government. An AF Form 1127, Hospital Invoice/Receipt/Accounts Receivable Record, will be prepared to ensure collection and/or billing of charges. If a contract employee cannot pay for the treatment at the time of discharge, a statement of charges and a letter of indebtedness will be completed and billed to the contract employee for medical services rendered. An accounts receivable record will be established for each contract employee who receives medical care and is unable to pay at the time of discharge. Follow-up on accounts receivable will be completed in accordance with AFI 41-120, Medical Resource Operations. After an accounts receivable record is established and billing for outpatient medical services has occurred, the SF 558, Medical Record - Emergency Care and Treatment, will be annotated with the date of billing and the voucher number and forwarded to Outpatient Records to be filed in the medical record folder. The contract employee shall ensure that he/she provides timely notice to his/her medical insurance company and that if the insurance company is to make payment, the insurance company has adequate information and documentation to make payment. A contract employee treated in a natural disaster is not charged for outpatient care. 4. Quality Control. a. The contractor shall insure that all work performed under this Statement of Work (SOW) meets the criteria established in paragraph 1. 5. Government Furnished Utilities and Services. a. The government shall provide, without cost to the contractor, the utilities (i.e., electricity, water) necessary to accomplish the task(s) specified in this Statement of Work, fire protection, and 911 services. 6. Installation Security a. Contractor shall adhere to the Installation Entry Control Procedures for Contractors as set forth in appendix "A" by 87th SFS, McGuire AFB, NJ. b. Antiterrorism: As a prelude and during performance of any contract on McGuire AFB, it is critical that the contractor be supportive of all DoD requirements to protect personnel, their families, installations, facilities, information, and other resources from terrorist acts. All contractors and sub-contractors are responsible for providing their personnel information on the McGuire Antiterrorism Program. During performance of work, personnel security and protection of resources is critical for the installation to maintain a safe work environment. Contractor personnel will be alert for any suspicious activity observed by their work force or any personnel working on the installation. If any situation appears to be suspicious, immediate actions must be taken to make proper notification to McGuire Law Enforcement by calling the McGuire Security Forces Desk at 609-754-2001. Both personnel and vehicles must be strictly controlled during performance of duty. No personnel and/or equipment will be allowed to be placed in harm's way that could aid or abet anyone with bad intention to cause harm to McGuire AFB. Any contractor doing construction work will adhere to the DoD Unified Facilities Construction Code Requirements. Contractors are encouraged to contact the McGuire Antiterrorism Office at (609) 754-1397 or 1611 for information regarding the installation Antiterrorism Program.
