- Solicitation No. VA-241-10-RQ-0644 The VA Boston Healthcare System intends to negotiate and procure on a sole source basis the service with American College of Physicians located at 190 N. Independence Mall West, Philadelphia, PA 19106. This procurement of the services includes vendor to provide the service of Annual Subscription to Medical Home Builder (a online subscription service where primary care managers can evaluate their PC practice and report to Veterans Administration Central Office). This procurement is being conducted under FAR 6.302-1, only one responsible source and no other vendor or service will satisfy agency requirements. To the best of our knowledge Be-Tech Inc is the only provider that can meet agency's requirements and standards. The NAICS 51120. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. The announcement constitutes the only solicitation; proposals are being requested and written solicitation will not be issued. The solicitation number is VA-241-10-RQ-0644 and is a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-43, effective September 23, 2010. A list of contract line items number(s), items quantities and units of measure are incorporated in the solicitation. The dates and place of delivery are included on the solicitation as well. The provision 52.212-2, does apply and established the criteria as Technical and past performance, when combined, are approximately equal to cost or price. Please include a complete copy of the provision at 52.212-3 (Offeror Representations abs Certifications - Commercial Items, with its offer. The provision 52.212.-4 is incorporated. B. 2 ACRONYMS B.3STATEMENT OF WORK Subscription to Primary Care Unit/Service Self-Assessment Tool "Medical Home" is a relatively new patient-centered care concept; a health care setting that facilitates partnerships between individual patients, and their personal physicians, and the patient's family. It also assures that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner. The "Medical Home" concept reflects the input of the American College of Physicians (ACP), American Academy of Family Physicians (AAFP) and others to assess whether physician practices are functioning based upon the joint principles developed by the primary care specialty societies resulting in the National Committee on Quality Assurance standards, which emphasize the use of systematic, patient-centered, coordinated care management processes. The Department of Veterans Affairs currently has no tool by which to evaluate the Medical Home/patient-centered care, as defined by the NCQA standards, being provided by the VA's Primary Care units. The Service Provider will provide access to an on-line subscription service for VA-wide Primary Care managers, VACO Office of Patient Care Services and the VACO Technology Assessment Program to do: 1.Assess Primary Care service against National Committee of Quality Assurance (NCQA) standards 2.Generate assessment information for quality improvement work planning at local, VISN and national VA levels. At present, electronic submission is voluntary and any nominal registration fees will be the responsibility of the contractor. VA intends to mandate electronic invoice submission, subject to completion of the federal rulemaking process. At present, VA is using a 3rd party agent to contact contractors regarding this service. During the voluntary period, contractors interested in registering for the electronic system should contact the VA's Financial Services Center at http://www.fsc.va.gov/einvoice.asp. The following factors shall be used to evaluate offers: Technical and past performance, when combined, are approximately equal to cost or price. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party.
