Description
Sole Source Justification Letter Performance Work Statement This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is F3Z4530147AQ03; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-44 effective 08 Jul 10, Defense DCN 20100820 effective 20 Aug 10, and AFAC 2010-0402 effective 02 Apr 2010. This acquisition is unrestricted; the North American Industry Classification System (NAICS) code is 541511. The business size standard is $25.0M. The Federal Supply Class (FSC) is J065. The Standard Industrial Classification (SIC) is 7371. TRAVIS AFB INTENDS TO AWARD A SOLE SOURCE PURCHASE ORDER FOR ALARIS PC SYSTEMS SOFTWARE MAINTENANCE AGREEMENT TO RDL REFERENCE LABORATORY. CLIN0001- Reference Laboratory to provide specialized, in-depth autoimmune disorder testing for David Grant Medcal Center. Period of Performance: Base Year 01 Oct 2010 - 30 Sep 2011 Option Year 1 01 Oct 2011 - 30 Sep 2012 Option Year 2 01 Oct 2012 - 30 Sep 2013 Option Year 3 01 Oct 2013 - 30 Sep 2014 Option Year 4 01 Oct 2014 - 30 Sep 2015 Please provide quotes valid until 1 October 2010. Award shall be made in the aggregate, all or none. The following provisions and/or clauses apply to this acquisition. The Government's obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer.
