Description
DEPARTMENT OF VETERANS AFFAIRS The Portland VA Medical Center (PVAMC) together with the Southern Oregon Rehabilitation Center and Clinics (SORCC), and the VA Roseburg Health System (VARHS) have a requirement to obtain contractor services necessary for Clinical Informatics and Care Management components of the VISN 20 Medical Home Demonstration Lab. Together, these facilities will implement and evaluate novel approaches to manage complex patient cohorts within a new Medical Home framework for improved patient care, outcomes, satisfaction, and resource utilization. The successful implementation will require the development of tools to enhance population management, care management, and communication between teams. The Contactor will work in collaboration with the VISN 20 Medical Home Demonstration Lab Project Team on the following: 1) assist in the development of an informatics tools to support care management activities of clinical staff, 2) assist in the development of strategies for comorbid patient assessment and risk stratification through integration with disease registry data, 3) provide specific training to clinical staff on care management techniques and tools through in-person training, web-based courses, and telephone and/or email support as needed, and 4) participate as a member of the VISN 20 Medical Home Demonstration Lab Advisory Committee to provide expertise on care management, medical home redesign, the effectiveness and dissemination of complex clinical redesign programs, and clinical informatics in relation to the grant objectives and progress of the Project Team in meeting those objectives. It is our intention to negotiate a sole source Firm Fixed Price Contract for a base and four one year options and will utilize procedures under FAR Part 12 and 13, with make award to Care Management Plus of the Department of Medical Informatics and Clinical Epidemiology, Oregon Health & Science University, Portland, OR. It is intended that this procurement will be sole source per 10 U.S.C. 2304 (c) (3) or 41 U.S.C. 253(c)(3) under the authority of FAR 6.302-3(a)(2)(ii) Industrial Mobilization, Engineering, Developmental or Research Capability, or Expert Services, in order to establish or maintain an essential engineering, research or development capability to be provided by an educational or other nonprofit institution or a federally funded research and development center. (See attached Justification for Other-Than-Full-And-Open-Competition) This is a very specialized field of scientific study of which they are a leading national expert and further meets the requirement of being located locally. Anticipated award date is 9/30/2010. The NAICS code 5471711, Size Standard is 500 employees. This notice of intent IS NOT A REQUEST FOR QUOTATIONS. Information received will be considered solely for the purpose of determining whether conducting a competitive procurement is in the best interest of the Government in the future. Interested parties may identify their capabilities by responding to this requirement and considered for future reference. All information submitted should support the capability to provide the items required, and shall be furnished at no cost or obligation to the Government.
