- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Quotations are being requested and a written solicitation will not be issued. Solicitation Number VA-259-10-RQ-0270 is issued as a Request For Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-44. This is an emergency competitive procurement under the North American Industrial Classification System (NAICS) Code 333999 and Small Business Size Standard of 500 employees'. The SAO Western Region, VISN 19 Consolidated Contracting Activity intends to contract for a vendor to provide to the VA Medical Center, 2121 North Avenue, Grand Junction, CO 81501, one (1) duplex assembly vacuum system. The pumps shall be a oil-free, positive displacement, non-pulsating water-ring type. The motors shall be NEMA rated open drip proof, 1800 rpm, 208 VAC, 3-phase. The assembly shall be provided with an ASME receiver with all components and subassemblies for water and seal medium. Contractor shall provide a control system with ability to lead/lag motors. Alarms shall annunciate both locally and remote. The controller shall be of a programmable logic type. The controls system shall include a timer. This assembly shall comply with the current edition of NFPA 99. Offeror may provide pricing via company letterhead. All pricing/costs shall include rapid/expedient transport to the delivery Grand Junction warehouse F.O.B. Destination. Upon award the contractor shall deliver the required product to the Grand Junction VA Medical Center within 7 calendar days after award. The provisions at FAR 52.212-1, Instructions to Offerors-Commercial items applies to this acquisition with the following changes, items (e) and (h) of the standard clause are deleted. Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial items, with their offer (for full text copies of clauses and provisions reference Internet Web Site - https://www.acquisition.gov/far/). The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition which shall include the following clauses: 52.219-28, 52.222-19, 52.222-21, 52.222-26, 52.222-36, 52.225-3 Alt (ii), 52.225-13, and 52.232-34. The following VAAR clauses are incorporated and apply to this acquisition (for full text clauses and provisions reference Internet Web Site - http://www1.va.gov/oamm/oa/ars/policyreg/vaar/): 852.203-70, 852.211-70, 852.246-70 which requires a contractor guarantee of the equipment against defective material, workmanship and performance for a period of 1 year, and 852.270-1.
