Description
Past Performance Questionnaire Statement of Work This is a combined synopsis/solicitation prepared in accordance with FAR SubPart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Offerors are responsible for monitoring this site for the release of any amendments or other pertinent information. This solicitation will be issued as a request for quotation (RFQ) under the solicitation number: F3A0T50237A002. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-25, Defense Federal Acquisition Regulation Supplement Change Notice (DCN) 2008-0513, and Air Force Acquisition Circular (AFAC) 2008-0605. The North American Industry Classification System (NAICS) code is 711510 and the business size standard is $7,000,000. This solicaition is for Base year and 4 option years. Please quote on all years This RFQ contains three (3) line items. The items are to be firm fixed priced in accordance with the Statement of Work (attached): 0001AA: Music Accompanist for Saturday Mass- 52 EA 0002AA: Music Accompanist for rehearsals for Saturday Mass- 52 EA 0003AA: Special Services- Holy Day Observations, Funerals, Mermorial Services, Seasonal Musical Special Programs- 24 EA The place of service, final acceptance, and FOB destination point is JB MDL, Lakehurst, NJ. Acceptance will be performed by the Government or a Government representative at designated government location. The Government anticipates the award of a firm-fixed price contract. The Government intends to award one contract as a result of this solicitation; however, the Government reserves the right to award no contract at all, depending on the quality of the offers submitted. All referenced Federal Acquisition Regulation (FAR) clauses and provisions in this solicitation may be accessed via the internet at https://farsite.hill.af.mil. 52-204-7 Central Contractor Registration (Apr 2008); with 252.204-7004 ALT A (Sep 2007) 52.209-6 Protecting the Government's Interest when Subcontracting With Contractors Debarred, Suspended or Proposed for Debarment (Sep 2006) 52.212-1, Instructions to Offerors - Commercial Items (Jul 2008), applies to this acquisition. Addenda to the following paragraphs of 52.212-1 are: (a) All quotes are due on or before 1000 hrs (10:00am), Eastern Standard Time (EST), Tuesday, 14 September 2010. Electronic quotes are acceptable. Submit quotes to Stephanie Simpson via email, stephanie.simpson@mcguire.af.mil, or fax, 609-754-1105. Quotes must reference the solicitation number. (b) Offeror shall be registered in the CCR database prior to award, during performance and through final payment of any contract resulting from this solicitation. If the offeror does not become registered in the CCR database in the time prescribed by the Contracting Officer, the Contracting Officer will proceed to award to the next otherwise successful registered offeror. In accordance with FAR clause 52.212-2 (a), Evaluation - Commercial Items (Jun 2008), the Government will award a contract resulting from this solicitation to the responsible offeror whose quotation conforming to the solicitation and whose offer will be most advantageous to the Government based on price, experience, and past performance. Of the evaluation criteria, the contractor's experience and past performance is significantly more important and weighted more heavily than price. Attached are the past performance cover letter and questionnaire. Please forward past performance cover letter and questionnaire to various companies you provided staff in the past. Have at least three companies return fully completed questionnaires directly to Stephanie Simpson (REF 52.212-1). Proposals submitted without the past performance information will be rated as "neutral" on this evaluation factor and still considered for an award. Past performances completed on alternative forms will not be accepted. Offerors must comply with the provision 52.212-3, Offeror Representations and Certifications - Commercial Items (Jun 2008) with Alternate I. An offeror shall complete only paragraphs (b) of this provision if the offeror has completed the annual representations and certificates electronically at http://orca.bpn.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (c) through (m) of this provision. 52.212-4, Contract Terms and Conditions-Commercial Items (Jun 2010) 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (Jul 2010), is applicable to this procurement.
