- REQUEST FOR PROPOSAL Medical Service The South Bronx Job Corps Academy is a Federally Funded Vocational and Educational Training Program that services youth between the ages of 16 and 24 in a residential setting. The program operates year round with approximately 200 residential and 75 non-residential trainees. The program is operated by ResCare, Inc. for the United States Department of Labor. The South Bronx Job Corps Academy is soliciting Dental Service. All interested parties must bring resume, list of references, and copy of Insurance, Registration and DEA (if applicable) Attached you Will find medical Bid Specification. Please read carefully, fill out and return no later than the COB September 20, 2010 Scope of Work The hour for service will be Twelve(12) hours a week. The selected party will able to arrange the days from Monday to Friday and time 8am to 5pm An mandatory walk-thru has been scheduled for South Bronx Job Corps Academy: Friday September 10, 2010 at 10:00am •· Please be advise that once the walk thru begins no one will be allowed in. REQUIREMENTS •· Must be in compliance with all Local, State and Federal Guidelines and Regulations •· Work to be accomplished in safe and efficient manner with minimal or no disruption or endangerment to trainees and/or staff. •· Must provide a copy of current resume, licensure and insurance LENGTH OF AGREEMENT The performance for this agreement will be from October 1, 2010 to September 30, 2011 All questions regarding this matter can be directed to Vivian Lopez, Purchasing Agent at 718-294-7530 or by email Lopez.vivian@jobcorps.org.
