- The Department of Veterans Affairs Medical Center, Salem, Virginia (VAMC Salem) is seeking potential sources to furnish all equipment, facility, professional and technical personnel services to provide routine x-ray films for eligible veteran beneficiaries as referred by the Staunton Community Based Outpatient Clinic (SCBOC) a part of VA Medical Center, Salem, VA. The Contractor's place of business must be within a close proximity of the city limits of Staunton, VA., in order to allow patients to receive radiology services in a timely, safe and efficient manner without compromising patient care. The SCBOC location will be in the town limits of Staunton, Va. The Contractor must possess all licenses, permits, accreditation's and certificates as required by law to perform imaging services. Contract services will be provided on an as needed basis and the contract Base Year period will be from Date of Award through January 30, 2012, subject to the availability of FY12 funding, with the option to extend the contract for four (4) one year Option Periods. It is the VA's intention to negotiate and award on a competitive basis, pending availability of funds. The contracted radiologists shall be ACR board certified or board eligible and be located within the United States. Patients will be walk in patients Monday thru Friday, between the hours of 8:00 am and 5:00 pm at the contractor's place of business. The patient will arrive at the contractor's place of business with an x-ray order request filled out with designated x-ray procedure, CPT code, appropriate clinical history and signed by the Staunton CBOC ordering clinician. Upon completion of the x-ray examination and the interpretation by the contractor's radiologist, the contractor will provide an original transcribed and verified report accompanied by all patient images on a CD stored in a DICOM (.dcm) format to the Salem VAMC within twenty- four (24) hours on urgent requests and (48) hours on routine requests. Per VA Directive 6609, these CD's must be sent to Salem VAMC via a secure delivery service that tracks mail from pick-up to delivery. The Contractor shall absorb all costs of shipping these reports and CD's to the Salem VAMC. Coverage must be provided on all federal holidays for 24 hours, to include coverage in the absence of any of the other providers due to sick leave, family leave or other personal emergency. Cost basis/reimbursement will be by Current Procedural Terminology (CPT) codes. Estimated annual number of patients is 720/yr. The NAICS code for this requirement is 621512. Size Standard is $132.5M. This is a sources sought notice only. No formal solicitation document exists at this time. Responses to this notice should include the following: name and address of company/provider, number of radiologists employed by firm; business size (large or small); identify whether or not firm falls into any of the following socioeconomic categories - 8(a), HUBZone, Service Disabled Veteran-Owned; firms experience in providing Radiology diagnostic imaging services. Information on radiology diagnostic imaging services for current and/or previously preformed for the VA Medical Center and/or other government entities to include contract numbers estimated annual number of procedures and coverage provided per week. Any other pertinent information which demonstrates firm's ability to meet the above requirement. Please note in order to do business with the Government the contractor has to be registered at the Central Contractor Registration (CCR) database at www.ccr.gov and have a valid DUNs and Bradstreet Number which can be requested thru the CCR website.
