- In order to speed up the process to access Fort Bliss for the Industry Day on 14 October 2010, please e-mail your personal and vehicle registration information to Mr. Joe Bob Martinez at Joe.Bob.Martinez@us.army.mil and Diana.Morris1@us.army.mil. They will process your request for a temporary pass so that when you arrive at the Pass Office located outside the gate on Sargents Major Blvd, all you will have to do is show your ID, Vehicle Registration, and Proof of Insurance to obtain the pass. They will need to provide the following information: VEHICLE REGISTRATION FORM FOR E-PASS (PLEASE TYPE IN ALL INFORMATION PRIOR TO SENDING TO JOE OR DIANA) (AR 190-5 and USAADACENFB regulation 190-5) (Privacy Act Statement on Reverse side) PERSONAL INFORMATION: LAST NAME: FIRST NAME: Middle Initial: Drivers License #: Drivers License State: Birth Date: Destination/Purpose: Gender: Height (ft/in): Weight (lbs): Eye color: Hair color: Race: Home Address: City: State: ZIP: Phone: VEHICLE INFORMATION : Year: Make: Model: Body Style: Color: License Plate #: State of Issue (Plate): Registration Expiration Date: Insurance Company Name: Insurance Policy Number: Ins Exp Date: DATA REQUIRED BY THE PRIVACY ACT AUTHORITY:Title 10, United States Code, Section 3012 (g) PRINCIPLE PURPOSE:To provide commanders and law enforcement officials with a means by which information may be accurately identified. ROUTINE USES:Personal information is for statistical use and analysis of vehicle registration requirements. Information serves as a record of vehicle registration IAW AR 190-5 and USAADACENFB Regulation 190-5. DISCLOSURE: Disclosure is voluntary.
