Description
Description of Project: ­­ ­­­­­­­­­­­­­ The Stanford Institute for Materials and Energy Sciences (SIMES) Project is moving forward for construction and renovation of existing buildings for new offices, laboratories and training center. The new facilities are to be constructed in or adjacent to Building 040. It is the intent to obtain a complete renovation of B040 third floor for offices, complete renovation of the partial (northern section) of the second floor of B040 for offices, renovation of the partial (northern section) of B040 Annex for laboratories, and a training center through renovation of an existing structure adjacent to B040 Annex. The total area of the project scope is approximately 17,100 square feet. (See also attached Statement of Work.) Major Technical Factors: Construction of laboratories equipped with fume hoods, HEPA filtration, various chemicals, lasers, compressed gases and controlled atmospheric conditions. Renovation of existing, occupied building including partial demolition, shared construction access with building occupants, safe construction practices (LOTO, work planning, management of trade contractors). Response Content Interested firms must reply on company letterhead and respond to each of the items listed below. There is no specific format for your responses. Insurance Requirements The following minimum levels of insurance are required for this project: Comprehensive General Liability: $5,000,000 Business Auto Liability (per occurrence) $1,000,000 The Subcontractor shall, at its own expense, provide and maintain insurance under a "Standard Builder's Risk Course of Construction" policy (for awards of $50,000 or more) and Workers' Compensation insurance as required under California State law. Provide a current certificate of insurance showing that the Subcontractor can meet the insurance requirements listed above. California Contractor License Provide license (s) and date of expiration. Financial Data Provide company Dun & Bradstreet report. If D&B is not available, provide most recent financial statements. •4. Surety Provide name of surety. Surety must be listed on the Department of the Treasury's Listing of Approved Sureties. Provide certification from the surety or its agent regarding: •- The number of times it has completed Subcontractor's work in the past five years, •- Total performance and payment bonding capacity of the Subcontractor, in dollars, and •- Available bonding capacity of the Subcontractor at time of response, in dollars. Safety Records Experience Modification Rates (EMRs): Provide a signed statement from the Subcontractor's Workers' Compensation carrier listing the Subcontractor's California EMRs for each of the last three years where data is available. Provide the Subcontractor's NAICS Code for OSHA reporting purposes. If the EMR is higher than 1.00 provide letter of explanation. Experience and Personnel •- Provide a brief descriptive listing of similar projects in size and complexity completed during the past five years. •- Provide value of the work performed by the Subcontractor's own forces on all projects listed above. •- Provide the name and phone number of the owner's representative for each project listed above. •- Personnel: provide resumes of project manager and on-site superintendent for the project •7. What is your experience in working on federal funded subcontracts? •8. What is your experience in working on DOE Laboratories or in highly regulated work environments? The University will evaluate submittals of completed responses to determine if, in the University's opinion, the Subcontractor is deemed qualified for successful performance of the type of work required for this project. The University reserves the right to reject any or all responses and to waive non-material irregularities in any response. All information submitted as a response will be considered official information acquired in confidence and the University will maintain its confidentiality to the extent permitted by law.
