- This announcement constitutes only the pre-solicitation notice; quotes are being requested and a written solicitation will be issued on or around 27 October 2010. Solicitation number AG-82A7-S-11-0004 is being issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 05-41. All applicable provision and clauses are included in the actual solicitation. This electronic announcement constitutes the only pre-solicitation notice being issued. THIS PROCUREMENT IS SET ASIDE 100% FOR SMALL BUSINESS. The North American Industry Classification System (NAICS) code is 621111, and the small business size standard is $10 Million. The Government intends to issue a firm fixed price contract for the following service: Physician Services IAQ SOW.IAW the Statement of work at Treasure Lake Job Corps Indiahoma, Oklahoma. Please reference solicitation # AG-82A7- S-11-0004 on all correspondence. All quotations must be received via e-mail (jjbartholomew@fs.fed.us or fax (303-275-5294) to Joel Bartholomew at the 740 Simms St Golden, CO 80401 by 20 Aug 2010 at 4:30pm (CST). Quotes must be valid for 30 days after the date that the quotes are due. NO PARTIAL AWARDS WILL BE MADE. Please acknowledge all requirements on quotations.
