- Quality Assurance Surveillance Plan Appendix A Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement and attached solicitation constitutes the only announcement for the solicitation. Quotations are being requested. This is a request for quote (RFQ) No. AG-02NV-S-11-0004; quotation due date is December 10, 2010. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 40, effective as of March 23, 2010. Attached, you will find a Statement of Work for the Janitorial Services for the Big Piney Ranger District, Bridger-Teton National Forest. No later than 3:00 pm MST, December 10, 2010 please provide a price and experience questionnaire related to the work described in the attached Statement of work. The selection process will consist of a Best Value source selection based on past performance and price. Under a Best Value source selection, non-price evaluation factors, when combined are significantly more important than price. However, the Contracting Officer will not select an offeror for award on the basis of a superior capability without consideration of the amount of the price. This procurement is a 100% Small Business Set Aside. The NAICS code for this solicitation is 561720. In order to be considered a small business under this NAICS code, your average annual receipts must not exceed $16.5 Million.
