Description
Please see attached Statement of Work For providing 40 Annual Firefighter Medical Examinations each fiscal year. The NAICS code is 621111 and size standard is less than $10.0 million. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26, effective 1 June 2008 & Class Deviation 2005-00001. Dates for Option Years 1-4 will be provided with Notice of Intent to Exercise Option, no later than 60 days prior to the effective date of the option. The total potential effective period of the award resulting from this solicitation is 60 months from date of award. The Contractor shall not be required to accept any new orders beyond 60 months from the award date. Regulatory Compliance: The contractor shall ensure the requirements of Occupational Safety and Health Administration (OSHA), HIPPA, and other applicable Federal regulations are met. Security Requirements: All contract employees shall adhere to all local and Air National Guard security regulations and directives. Invoicing and Payment: This contract will have one CLIN with a total number of physicals to be performed during the Fiscal Year. An invoice may be submitted upon completion of physicals. Instructions to Offerors Offerors shall provide a firm, fixed price for health care services based upon the following schedule of items and prices: Base Year CLIN Description Unit 0001 Annual Firefighter Medical Examinations 0002 Annual Firefighter Medical Examinations Option Year 1 0003 Annual Firefighter Medical Examinations Option Year 2 0004 Annual Firefighter Medical Examinations Option Year 3 0005 Annual Firefighter Medical Examinations Option Year 4 Contractor is authorized travel and per diem in accordance with the JTR. 52.212-2, Evaluation-- Commercial Items (January 1999). The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government. The following factors shall be used to evaluate offers: price, past performance, and the following technical factors in descending order of importance: 1) The contractors ability to meet the requirements in the above Performance Work Statement; and 2) Contractors experience with and knowledge of Army National Guard behavior health operations and regulations. The technical evaluation factors listed above, and past performance, when combined are approximately equal in value to Price. Offerors shall ensure that the above technical factors are addressed in their proposals.
