- Biological Hood Testing and Certification RFP No. VA-248-11-RP-0025 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a request for proposal (RFP). This solicitation document and all incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-46. This is a total small business set-aside. The relevant NAICS code is 621511 with small business size standard of $13.5 million in total average annual receipts. Proposals are due on November 30, 2010 at 5 p.m. Eastern Time. Proposals, as well as any questions or requests for clarifications, should be submitted to Eric Sullivan, Contracting Officer at the West Palm Beach VA Medical Center, by email (eric.sullivan2@va.gov) or facsimile (561-422-6507). The West Palm Beach VA Medical Center (7305 North Military Trail, West Palm Beach, FL 33410) has a requirement for semiannual (typically June and December) testing and certification services for the biological hoods (items 1 - 15) and air sampling particle count testing (item 16) listed below. The desired period of performance for semiannual testing and certification is as follows with the testing and certification first occurring in December 2010 by request of the West Palm Beach VA Medical Center's Biomedical Service. Base period: December 2010 - November 2011 (testing/certification done twice per year) Option period 1: December 2011 - November 2012 (testing/certification done twice per year) Option period 2:December 2012 - November 2013 (testing/certification done twice per year) Option period 3:December 2013 - November 2014 (testing/certification done twice per year) Option period 4: December 2014 - November 2015 (testing/certification done twice per year) Award shall be made to the responsive, responsible offeror determined to have the lowest priced, technically acceptable offer. Successful offers must be registered with the Central Contractor Registration (https://www.bpn.gov/ccr/default.aspx) and provide representations and certifications either electronically online (https://orca.bpn.gov/) or in writing by obtaining a written form by request from the Contracting Officer. The Service Contract Act may apply to this procurement. The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. Each offeror shall complete the following certification in accordance with FAR 52.222-52: The offeror __ does __ does not certify that: (1) The services under the contract are offered and sold regularly to non-Governmental customers, and are provided by the offeror (or subcontractor in the case of an exempt subcontract) to the general public in substantial quantities in the course of normal business operations; (2) The contract services are furnished at prices that are, or are based on, established catalog or market prices. An "established catalog price" is a price included in a catalog, price list, schedule, or other form that is regularly maintained by the manufacturer or the offeror, is either published or otherwise available for inspection by customers, and states prices at which sales currently, or were last, made to a significant number of buyers constituting the general public. An "established market price" is a current price, established in the usual course of ordinary and usual trade between buyers and sellers free to bargain, which can be substantiated from sources independent of the manufacturer or offeror; (3) Each service employee who will perform the services under the contract will spend only a small portion of his or her time (a monthly average of less than 20 percent of the available hours on an annualized basis, or less than 20 percent of available hours during the contract period if the contract period is less than a month) servicing the Government contract; and (4) The offeror uses the same compensation (wage and fringe benefits) plan for all service employees performing work under the contract as the offeror uses for these employees and for equivalent employees servicing commercial customers. (b) Certification by the offeror as to its compliance with respect to the contract also constitutes its certification as to compliance by its subcontractor if it subcontracts out the exempt services. This service shall include items such as cleaning, lubrication, calibration, inspection, testing and parts. 3) Contractor shall provide evidence of factory training, either specific to the manufacturer or on Biological hoods in general, for each service engineer upon request. III.PROGRESS AND COMPLIANCE A.Vendor shall provide evidence of factory training for each service technician upon request. B. Contractor shall provide certification worksheets on each machine tested. C. Contractor shall contact Biomedical Instrumentation, (561) 882-6658 to arrange for service. D. Service reports shall be provided to Biomedical Engineering within 24 hours of completing service. The report shall include complete details of all work performed. E. The contractor shall provide certification in the form of technical documentation, original invoice, or service documentation that the quality of updates and upgrades is sufficient to guarantee continued performance and operation of the system listed in this contract.
