Description
This attachment includes statement of work, all required documents and clauses. This is a combined synopsis/soliciation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This procurement is being conducted under Simplified Acquisition procedures pursuant to the authority of FAR Subpart 13.5, Test Program for Certian Cermmerical Items (10 U.S.C. 2304(g) & 2305 & 41 U.S.C. 253 (g) & 253a & 253b) FAR Part 12, Acquisition of Commerical Items (title VII of the Federal Acquisition Streamlinig Act of 1994)(Public Law 103-355) & FAR 37.4. This announcement constitutes the only soliciation; proposals are being requested and a written soliciation will not be issued. The Aberdeen Area Indian Health Services (IHS) intends to award a firm-fixed price non-personal healthcare services type contract in response to Request for Quotes (RFQ) 11-065. This award is for one year, from the date of award to 12/31/11. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-38. This is a small business set-aside. The associated North American Industry Classification System Code is 621512. PURPOSE OF CONTRACT: See attached Statement of Work: Location is at the Pine Ridge IHS Hospital, E HWY 18, Pine Ridge, SD 57770 Ultrasound Services- The quoted unit pricing must be an all inclusive cost(to include but not be limited to transportation, food, lodging, per diem and fringe benefits). The contractor is responsible for all associated costs for the Ultrasound Services. FAR 52.212-1 Instructions to Offeror-Commerial Items (June 2008) 1. Solicitation number; 2. Closing date of December 30, 2010 at 12:00pm CST 3. Name, address, telephone number of the offeror and email address of the contract person; 4. Technical description of the item/service being offered in sufficient detail to evaluate compliance with the requirements of this solicitation. This may include product literture, or other documents, if necessary; 5. Terms of any expres warranty; 6. Price and any discount; 7. "Remit to" address, if different than mailing address; 8. A completed copy of the representations and certifications at FAR 52.212-3; 9. Acknowledgement of Solicitation Amedments; 10. A statement specifying the exten of agreement with all terms, conditions, and provisions included in the solication. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration. The Contractor must perform in accordance with Section 231 of the Crime Control Act of 1990, P.L. 101-647. Failure to complu with these statutes will result in immediate removal. Prior to award a Childcare National Agency Check and Inquiries (CNACI) suitability screening will be performed and cleared according to the information provided to us on the Optional Form 306. The individual performing must complete and return the following forms to the above Acquisition Official as part of their package: - OF-309 Declaration for Federal Employment - OF-306 Addendum to Declaration for Federal Employment - Agreement to a temporary waiver of character investigation - Licensure Agreement The selected offeror must make arrangement with the Service Unit Background Investigation Coordinator and complete the following as soon as award is made: - Fingerprints (done electronically) - SF-85 Questionnaire for Non-Sensitive Positions (done electronically)
