- Description
- The Phoenix Area Indian Health Service (PAIHS), Phoenix, Arizona, is soliciting proposals from qualified Buy Indian firms who qualify under the authority of the Buy Indian Act 25 U.S.C. 47, to provide all necessary resources, personnel, pharmaceutical drugs, supplies, equipment, facilities and other items and expertise necessary for providing medically necessary air medical transportation services for patients referred by the Hopi Health Care Center, Polacca, Arizona; San Carlos PHS Indian Hospital, San Carlos, Arizona; Bylas Health Center, Bylas, Arizona; Whiteriver PHS Indian Hospital, Whiteriver, Arizona; Cibecue Health Center, Cibecue, Arizona; Parker Indian Health Center, Parker, Arizona; Peach Springs Health Center, Peach Springs Health Center, Peach Springs, Arizona; and Supai Health Center, Supai, Arizona. In addition to other factors, reimbursement will be based on fixed rates per flight by Fixed Wing and Helicopter to various destinations as specified in the bid schedules. The fixed rate shall be inclusive of base rate, mileage rate, medical supplies, oxygen, and all necessary services required in transporting the patient safely to their destination. The period of performance shall be for one (1) base year with four (4) option years.
