PLACE OF PERFORMANCE: VA MEDICAL CENTERS LOCATED IN VISN 2. BUFFALO, NY, SYRACUSE NY, ALBANY NY. AGENCY: Department of Veterans Affairs POC: MARY T. BETKER DESCRIPTION: The Department of Veterans Affairs, VISN 2 MEDICAL CENTERS intends to negotiate a sole source procurement under the authority of FAR 6.302.1, with Immunodiagnostics Systems Inc, 17029 E. Enterprise Dr., Fountain Hills, AZ 85269, to procure Vitamin D 25-Hydroxy Kits for the facilities to perform Vitamin D testing. Interested parties may identify their experience in providing these kits, to the Contracting Officer via e-mail at mary.betker@va.gov or fax 716-684-0474. A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA-528-11-RP-0062 and issued as a request for proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-45, effective December 16, 2010. The associated NAICS code is 424210 and small business size standard is 100 employees. The acquisition is not set-aside and is being negotiated as a sole source procurement. Contract Term: 1/1/2011 - 12/31/2011. This is a Firm Fixed Price Contract. Price is to include shipping. Quantity is estimated. The requirement for the Immunodiagnostic Systems Inc Vitamin D Kits include the following: 1. Vitamin D 25-Hydroxy Kit ITEM EST. 650 KITS (96 WELLS/KIT) QTY - 1 UNIT - KIT UNIT COST/KIT $___________TOTAL COST $____________ The date of delivery is ordered by purchase card with freight pre paid + added with delivery being FOB Destination to the Department of Veterans Affairs Medical Centers located as above. The provision at FAR 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition along with additional addenda.
