- This document is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, proposals are being requested, and a written solicitation will not be issued Interested persons may identify their interest and capability to respond to the requirement or submit proposals. This notice of intent is a request for competitive quotes, and all offers received prior to Jan 05, 2011 will be considered by the Government. NAICS code is 325413 and the size standard is 500 employees. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-46. Only emailed requests received directly from the requester are acceptable. EVALUATION FACTORS Offerors will be evaluated to the extent they can provide the items listed in the attached price schedule. The Government will award to the lowest price, responsible, technically acceptable offeror. The technical factors to be evaluated are: I.Providing a brand name or equal item to those listed. II.Shelf life of items (expiration dates) (Equal to or better than the items listed in the price schedule). INSTRUCTIONS TO OFFERORS Offerors shall use the attached price schedule to indicate the unite prices for the base and all option periods. Offerors are encouraged to fill out this price schedule electronically. Offerors must indicate the country of origin for their products. All Offerors shall list the average shelf life for each item being offered. If not providing the brand called for in this RFQ, the offeror shall provide documentation detailing the specifications of the product being offered as to how it meets the salient characteristics. Offerors shall indicate the manufacturer name, the brand, and the product number for each item that is not brand name. To the maximum extent practicable, offerors are encouraged to respond to this solicitation using pre-existing materials (such as brochures, catalogs, marketing documents) rather than creating new documents. Offerors shall annotate the salient characteristics listed above to indicate the location of the relevant information used to meet the specification (document name and page number).
