- Scope of Work Contractor shall provide an Employee Assistance Program (EAP) for the Pacific Northwest Region, the Pacific Northwest Research Station, Job Corps Centers located in Oregon and Washington; for those employees and members of immediate families who have career/vocational, physical and mental/emotional, financial/legal issues, anger management, depression, grief, alcohol, family/parenting, marital/relationship and drug related problems that impact or may impact employee conduct and/or work performance. This professional service encompasses local in-person EAP assessment, referral counseling, and brief treatment - up to three sessions. Actual treatment remains the employee's responsibility. The Program includes: 1. PROGRAM FAMILIARIZATION & ORIENTATION OF EMPLOYEES TO EAP CONTRACTOR: The Contractor shall provide, upon request, employee orientation sessions annually for every 250 employees, and managerial/supervisory orientation sessions annually for every 500 employees. 2. BASIC EAP SERVICES: EVALUATION, DIAGNOSIS, REFERRAL AND MANAGEMENT CONSULTATION AND ASSISTANCE: a) Upon request of an employee or supervisor, the contractor shall provide local and in-person assessment, referral counseling and brief treatment counseling services. b) The contractor shall make available to employees/supervisors a 24-hour, 7 day a week toll free access via the telephone and have the ability to communicate with Forest Service via electronic computer network. The contractor shall provide TDD/TTY access if the need arises. c) The contractor shall provide a website for online EAP information and tools. d) The contractor shall provide upon request of the employee or supervisor, a female or male counselor, one of which shall be fluent in Spanish. If the employee/supervisor is not comfortable the assigned counselor, at least an alternative provider shall be substituted. Interpreters in other languages shall be made available upon request. e) The contractor shall provide management consultation and referrals. f) Contractor shall provide client follow-up and satisfaction surveys. 3. SUBSTANCE ABUSE PROFESSIONAL SERVICES: At least one principal or staff counselor shall be a Substance Abuse Professional (SAP) available to employees covered by the commercial driver's licensing requirements found at 49 CFR Part 382 and sub parts. A SAP is a licensed physician (Medical Doctor or Doctor of Osteopathy); or a licensed or certified psychologist, social worker, employee assistance professional; or an addiction counselor (certified) NAADAC or by the ICRC/Alcohol and other Drug Abuse. A SAP shall have knowledge of, and clinical experience in, the diagnosis and treatment of alcohol and controlled substances-related disorders. The SAP shall provide employee and employer services as described in the U.S. Department of Transportation's June 1995 publication entitled, "Substance Abuse Professional Procedures Guidelines for Transportation Workplace Drug and Alcohol Testing Programs", or any updates thereto. 4. CRITICAL INCIDENT DEBRIEFINGS: The Contracting Officer may order critical incident debriefings, on a fee for service basis, due to a given crisis at any of the locations covered by the contract. 5. QUARTERLY UTILIZATION REPORTS: The Contractor shall provide quarterly utilization reports to the Contracting Officer Representative. 6. PROGRAM MATERIALS AND LITERATURE: The Contractor shall provide an employee promotional brochure annually for each employee. 7. LIABILITY: The Contractor shall assume all responsibility for legal and professional liabilities in rendering services, and shall submit evidence of liability insurance. This includes any and all insurance requirements as set by Federal, State and local agencies, normal to this environment. 8. PUBLICATION OF DATA: The Contractor shall not publish or disseminate any data obtained, or information received or developed from work pursuant to this contract without prior written approval of the Contracting Officer. 9. RECORDS: The Contractor shall maintain individual case files for each employee referred for counseling. Case files shall be maintained in accordance with the confidentiality requirements or PL 93-282 and implementing Federal Regulations of 42 CFR Part 21, and the Privacy Act of 1974, PL 93-579.
