- Performance Work Statement (Attachment 1) Request for Proposal 2011 Bid Table (Attachment 3) 2012 Bid Table (Attachment 4) Performance Plan (Attachment 2) This is a combined synopsis/solicitation for Sampling, Analysis and Monitoring of Wastewater, Stormwater, Groundwater and Hazardous waste at Beale AFB, CA. Refer to the attached Performance Work Statement for more information. Proposals are due December 9, 2010, 5:00 PM, Pacific Time. At a minimum, proposals must include the following documents: (1) Pages 1-7 of the solicitation, completed and signed. Pages 3-7 must be filled-out as follows: Provide Unit Prices and Amounts for the following item numbers only: 0001, 0002 and 0004 1001, 1002 and 1004 Note: Leave item numbers 0003, 0005, 1003 and 1005 blank. The government will insert dollar amounts when the contract is awarded. (2) Bid Table 2010 (Attachment 3) and Bid Table 2011 (Attachment 4). A one-time site visit of sampling locations has been scheduled for December 6, 2010 at 10:00AM for those interested. Refer to the solicitation for further instructions.
