Description
This is a pre-solicitation notice. The Federal Bureau of Prisons intends to make a single award to Solicitation RFQP0603110002 to a responsible entity for the provision of Optometrist Services in accordance with the statement of work for the inmates incarcerated at the Federal Detention Center located at 351 Elliott Street, Honolulu, Hawaii. The North American Industry Classification System (NAICS) Code is 621320 (Size Standard: $7.0 million). The requirement will be in accordance with standards and methods generally accepted within the field of optometry, and compliance with all Federal State policy and regulations. The contractor shall adhere to all standards and regulations associated with the Joint commission on Accreditation of Healthcare Organizations (JCAHO) and American Correctional Association Foundation/Core Standards for Adult Correctional Institutions. The contractor will not exceed 18 sessions of service per month with a session being defined as one (1) hour in duration. The requirement calls for approximately 6 sessions of service per week. Required working hours are normally between the hours of 7:00 a.m. and 1:00 p.m., Monday through Friday. Actual work hours may vary contingent upon the needs of the institution. Proof of qualification will be required when responding to solicitation. The solicitation will be distributed solely through the General Services Administration's Federal Business Opportunities web site 1. http: //www.fedbizopps.gov/. Hard copies of the solicitation will not be available. This site provides all downloading instructions to obtain a copy of the solicitation. All future information about this acquisition, including solicitation amendments, will be distributed solely through the referenced web-site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition. Faith-Based and Community-Based Organizations can submit offers/bids/quotations equally with other organizations for contracts for which they are eligible. Any contractor interested in receiving a contract award with the Federal Bureau of Prisons must be registered at www.ccr.gov and http: //orca.bpn.gov. The successful individual/organization providing optometry services under this contract award must have graduated from an accredited school or program; or, have a minimum one year of experience as an optometrist; duly licensed and must provide evidence of current certification as an optometrist. Contractors must at all times through the length of this contract maintain proper levels of malpractice liability insurance stated within the contract. The Contractor must specifically identify the individual that will be performing these services under any subsequent contract award. The Contractor must provide proof that their candidate meets all qualifications. The candidate's name and qualifications must be submitted at the time the quote is submitted. Contractors must submit with their quote a resume, photocopies of documents evidencing education and training. The successful contractor shall be responsible for all applicable federal, state and local taxes. There are no tax exemptions. This is a 100% small business set-aside. All responsible small business sources are encouraged to submit a quotation which will be considered for award. The solicitation will be available on or about February 11, 2011 with an anticipated due date for the receipt of quotes of February 25, 2011 at 2:00 p.m. The Government reserves the right to award without discussion, therefore, each initial quote should contain the quoter's best term from a cost or price standpoint.
