THIS IS NOT A FORMAL REQUEST FOR PROPOSAL (RFP) AND DOES NOT COMMIT THE CENTERS FOR MEDICARE AND MEDICAID SERVICES (CMS) TO AWARD A CONTRACT NOW OR IN THE FUTURE. This is a SOURCES SOUGHT NOTICE to determine the availability of 8(a) companies that are qualified to support CMS in providing for Warehouse, Transportation, Labor Support, Furniture Installation, Furniture Repair, and Lock and Key Services. Purpose of Contract: The purpose of this contract is to obtain ongoing warehousing, transportation, labor support, furniture installation, furniture repair and lock and key services throughout the CMS headquarters complex and various operational activities within the Baltimore/Washington metropolitan area. General Contract Requirements: Independently and not as an agent of the Government, the Contractor shall furnish all the necessary services, supervision, labor, materials, equipment and vehicles not otherwise provided by the Government, to provide ongoing warehousing, distribution, inventory, moving, labor support, locksmith, recycling, and furniture installation/repair services in an orderly, timely and efficient manner as required by the Centers for Medicare and Medicaid Services (CMS). The Contractor shall provide qualified personnel to perform a wide variety of warehousing, transportation and labor support services. These services shall be performed at the CMS Headquarters Complex located at 7500 Security Boulevard, Baltimore, Maryland 21244 and various other locations throughout the Baltimore/Washington area as required. The contractor shall provide competent supervisors with sufficient knowledge of the services required to insure performance by labor in accordance with good commercial practices. Supervisors shall be responsible for contractor performance and employee conduct and act as the Contractor's duly assigned representative at CMS. Labor Category Minimum Qualifications: The estimated number of labor employees per any eight (8) hour shift which may involve more than one (1) job assignment is as follows. The estimated requirements are not to be construed as a guarantee of actual requirements, as the Government does not guarantee any specific volume of warehousing and labor support services. One on site supervisor One inventory specialist Eleven warehouse specialist Two Lift Gate Truck Drivers Five laborers Vehicles - Equipment Requirements: One cargo van and one 25' 5 ton truck with lift gate required to be used as needed for delivery, transport and transfer services on a daily basis. All preventative maintenance and/or repairs to Contractor provided vehicles and equipment are the sole responsibility of the Contractor. Maintenance schedules for all vehicles are required to be in compliance with designated periodic Maryland vehicle inspection rules and regulations. In addition, the contractor shall perform weekly vehicle and equipment inspection/maintenance activities and all equipment assigned to this contract and prepare and submit an inspection/maintenance report to the CMS project officer within two working days of this activity. Reporting Requirements - Weekly Activity Production Reports: All work activities performed under this contract shall be documented using various warehouse/mover/furniture services work tickets. All completed activities shall be tracked and documented on a weekly contractor's activity production report submitted to the CMS project officer each Wednesday recording activities for the prior week. Work Hours Contractor shall provide personnel to be available to perform on a continuing basis, day-to-day, until a specific job assignment is completed. Normal working hours for all contractor personnel shall be between the hours of 7:00 a.m. to 6:00 p.m. Monday through Friday excluding legal holidays and administrative leave allowances provided to CMS employees. The Government reserves the right to request the contractor stagger starting times of contractor personnel to assure adequate coverage throughout the workday. The Government reserves the right to request Contractor personnel work mandatory overtime in emergency situations. While prior notification of these requirements will be attempted, there may be times when little or no advance notification can be provided. Overtime hours shall be determined and approved by the CMS project officer or his/her designated alternate representative. List of Contractor Requirements: The following specific information is requested as part of the contractor's response: 1) company descriptive literature; 2) specific related corporate experience; 3) experience with the type of performance expectations mentioned above; 4) references (to include a point of contact and phone number) with first-hand knowledge of the interested contractor's experience in the work described above; and 5) the business information outlined below. Documentation showing an audit of the contractor's accounting system showing the system is acceptable to conduct cost-type contract work (if such an audit has been done). 6. Point of Contact, phone number and email address of individuals who can verify the demonstrated capabilities identified in the responses. Teaming Arrangements: Entities seeking to respond to this notice as a team or to rely on subcontractors to perform any portion of the work must include the above-requested information and certifications for each entity on the proposed team or each proposed subcontractor. Teaming arrangements are encouraged. The contractor and any subcontractors must not have any relationships or arrangements through business operations or its employees that could be considered as possibly lessening the company's objectivity concerning any aspect of the proposed tasks. If such relationships or arrangements exist, contractors or subcontractors shall be required, during the procurement process (if one occurs), to identify potential conflicts of interest and discuss how the conflicts will be addressed and mitigated. This sources sought is for information and planning purposes and is not to be construed as a commitment by the Government. This is not a solicitation announcement for proposals and no contract will be awarded from this announcement. No reimbursement will be made for any costs associated with providing information in response to this announcement or any follow-up information requests. Respondents will not be notified of the results of the evaluation. All information submitted in response to this announcement must arrive on or before the closing date. Responses shall be limited to 10 pages. Resumes of key people are limited to 2 pages and may be submitted as an attachment. Attachments will not count towards the page limit.
