Description
PERFORMANCE WORK STATEMENT FOR CRUSHING AND PRODUCING AGGREGATE FROM QUARRY AT POHAKULOA TRAINING AREA, ISLAND OF HAWAII, HI 1. DESCRIPTION OF SERVICES. The Contractor shall furnish labor, equipment, transportation, utilities, and materials for the production of 6", 4", 2 ", 1 " minus with fines, 3/8" chip clean, #4 sand and " base coarse crushed aggregate at the PTA Quarry (TE#01). All work shall be performed in accordance with safety commercial practices and State, County, and Federal Standards. This is an IDIQ type contract. Estimated quantities are stated for the information of the prospective contractor. The actual quantity ordered may be more or less than the estimated quantity stated. As the prerequisite to the placement of any order and any liability of the Government hereunder, there must be (i) adequate and sufficient funds available to cover the order, and (ii) an administrative determination by the Government that the requirement exists. Task Order. The Contracting Officer's Representative (COR) or the Authorized Individual (AI) will order the work by submitting a task order either by email or by FAX to the Contractor. The Contractor shall submit to the COR or AI a confirmation by email of receipt of the order within 24 hours of receiving the task order. 1.1. Crushing and Producing Aggregate: Crush and produce aggregate in the sizes of 6", 4",2 ", 1 " minus with fines, 3/8" chip clean, #4 sand and " base coarse aggregate and stage at FP 427 and KeKee Quarry. 1.2. ROCK BLASTING/CRUSHING 1.2.1. The Contractor shall provide an automated portable or mobile rock crusher, equipment, materials and operator(s) to conduct crushing of the basalt rock to meet the specified size and gradation. 1.2.2. The rock crusher shall be equipped with a certified belt scale on the stacking conveyor and shall be calibrated to provide accurate weight of crushed rock as it is produced. The Contractor shall submit a copy of the certification for the scale to the COR prior to mobilizing the rock crusher. 1.2.3. All rock shall be crushed, screened, sifted or sieved, as required to meet the target gradation. 1.2.4. The Contractor shall produce crushed rock in the target gradations. The Contractor shall take samples and test the gradations for every 6,000 tons produced or a minimum once per week of production. Test results shall be provided to the COR every week. If the actual gradations are not within the target ranges, the Contractor shall adjust his process to attempt to bring the material into the target range by the start of the next week. The sampling and adjustment process shall continue each week of production. 1.2.7. The Contractor is responsible to provide licensed/certified personnel, supplies and materials to perform blasting. The Contractor shall coordinate with the COR on the start date. 1.2.8. The Contractor shall provide an Automated Portable Rock Crusher and operators to conduct crushing of dense blue rock to produce crushed rock at the PTA Quarry. Tonnage and size will be specified by the COR per requirements of each request. 1.2.9. All rock crushing operation shall meet all requirements of Department of Health (SDOH).. 1.2.10. The Contractor shall provide the COR a weekly report on the amount of rocks crushed daily. The report shall be provided before close of business on Mondays or first day of business following Federal Holidays. 1.2.11. The Contractor shall provide all equipment to stock pile and load crushed rock into delivery trucks. 1.2.12. The Contractor shall store all explosives off work site. 1.2.13. The Contractor shall provide dust control measures for crushing operation and stock piling, in accordance with the Clean Air Act. 1.3. PERMITS: Permits shall be acquired, maintained and renewed as needed. 1.3.1. The Contractor shall be responsible for obtaining and maintaining all necessary County, State, and Federal permits and licenses, to include the State of Hawaii Covered Source permit. 1.3.2. The Contractor shall obtain an individual storm water permit from the State Department of Health (SDOH) prior to quarry operation. The permit shall require the Contractor to put in place Best Management Practices (BMPs) to prevent polluted runoff from leaving the quarry site. 1.3.2.1. The Contractor is advised that the previous military operation of the rock quarry required annual storm water monitoring for the following constituents. Salinity, Turbidity, Total Suspended Solids (TSS), Biological Oxygen Demand (BOD), Chemical Oxygen Demand (COD), Ammonia, Nitrate+Nitrite, Total Kjeldahl Nitrogen, Total Phosphorus, Oil & Grease. During operation, there were permit exceedances in Ammonia, Nitrate+Nitrite, Total Kjeldahl Nitrogen, and Total Phosphorus. 1.3.2.2. The Contractor shall operate the quarry as not to exceed any of the limits placed within the new permit. There is currently an administrative extension for the permit on record at SDOH. The SDOH has indicated several times that they intend to place additional requirements to reduce TSS and nutrients (Ammonia, Nitrate-Nitrite, Total Kjeldahl Nitrogen, Total Phosphorus). By accepting this contract, the Contractor is agreeing to also implement 1.3.3. The Contractor shall submit an annual Discharge Monitoring Report to the SDOH. Contractor shall provide a copy of the report to the Contracting Officer Representative (COR). The COR will provide a copy to the Department of Public Works, Environmental Section. 1.3.4. The Contractor shall obtain a Clean Air Permit from SDOH for quarry operations and be compliance with all federal and SDOH Clean Air regulations. 1.3.5. The Contractor shall provide a copy of all permits to the COR within two (2) working days after received. The COR will provide a copy to the Department of Public Works, Environmental Section. 1.4. Business Relations. The Contractor shall partner with the Government to resolve all issues and conflicts within (2) business days. 2. SERVICE PERFORMANCE SUMMARY. The Contractor's service requirements are summarized into performance objectives that relate directly to mission essential items. The Contractor shall develop metrics for each objective listed below. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success. GOVERNMENT-FURNISHED PROPERTY AND SERVICES. 3.1. No utilities will be furnished by the Government, unless stated otherwise in the contract documents. No other labor, tools, equipment, supplies, materials, articles or transportation will be furnished by the Government, unless stated otherwise in the contract documents. 4. GENERAL INFORMATION. 4.1. Quality Control. Maintain a quality control program to ensure services are performed in accordance with commonly accepted commercial practices. 4.2. Quality Assurance. The Government will evaluate the contractor's performance under this contract using th Quality Assurance Surveillance Plan. 4.3. Contracting Officer's Representative (COR). Will be appointed by the Contracting Officer. 4.4. Hours of Operation. Perform work during Government's normal working hours, 7:30 a.m. - 4:00 p.m., Monday through Friday, excluding Federal holidays. In no event shall the Contractor perform work outside the normal working hours without obtaining written approval from the COR. 4.5. Spills 4.5.1. Contractor vehicles and machinery operating on the installation shall be free of any leakage including fuel, oil, hydraulic oil, transmission fluid, or any other leakage. Contractor is required to call in all spills/releases immediately to the DPW Emergency Spill Response Line at 808-656-1111 and submit written notification as soon as possible using the Spill Notification From in accordance with the 25th ID (L) & USARHAW Spill Prevention, Control, and Countermeasures Plan. 4.5.2. The Contractor shall be responsible for accomplishing all regulatory notifications to the State Department of Health Hazard Evaluation and Emergency Response (HEER) Office, Local Emergency Planning Committee, National Response Center, and/or Environmental Protection Agency (EPA), as applicable. Copies of written notifications shall be provided to the DPW Environmental Emergency Planning and Community Right-to-Know Act Program Manager, fax 808-656-1039 or STOP 253. 4.5.3. The Contractor shall be responsible for cleanup of any oil or hazardous substance spills or leakage to regulatory standards. Cleanup must be to the satisfaction of the Government and must be accomplished within two (2) days from the time leakage occurs at no additional cost to the Government. 4.6. Schedule. The Contractor shall coordinate a work schedule with the Contracting Officer's Representative (COR) within five (5) business days after award. 4.7. Work Site Cleanup and Disposal of Waste. Remove and dispose of all debris/waste at the end of the workday from work site per applicable federal, state and local requirements. 4.8. Identification of Contractor Employees. 4.8.1. Provide each employee with identification (ID) badge made of nonmetallic material, easily readable and including employee's name, contractor's name, functional area of assignment, with recent color photograph of the employee. 4.8.2. All contractor personnel shall wear the ID badge at all times when performing work under this contract at a Government site, including while attending Government meetings that may take place outside the Government facility. Contractor employee shall wear the ID badge in a conspicuous place on the front of exterior clothing above the waist except when safety or health reasons prohibit such placement. 4.8.3. All Contractor vehicles shall be identifiable with a sign bearing the company's name. 5.CONTRACTOR MANPOWER REPORTING (CMR). The CMRA is a business process that has been approved by the Army as a means for collecting information on contracts that provide services to the Department of the Army. Contractors are required to register and report information via the CMRA website located at https://cmra.army.mil/. Additional information such as links to user guides, frequently asked question and help desk information is available via the CMRA website. Reporting is required on an annual basis and shall be entered via the CMRA website. Reporting may be done at any time during the period of performance but not later than the completion of the contract or the Government Fiscal Year (FY), 1 October through 30 September, whichever occurs first. The report must be submitted NLT 30 Oct of each year. Data must be accurate and complete and shall be reported separately for each FY that falls within the period of performance. 6.TECHNICAL EXHIBITS TE #01 - Map of PTA Quarry
