Description
This is a SOURCES SOUGHT NOTICE (not a pre-solicitation notice pursuant to FAR Part 5) is to determine the capability of potential sources and the availability of commercial sources pursuant to FAR Part 10. This notice is issued by the Department of Veterans Affairs VA MidSouth Healthcare Network (VISN 9) on behalf of The Veteran Affairs Medical Center (VAMC) Nashville, VAMC Murfreesboro, VA Outpatient Clinic Chattanooga, and VA Outpatient Clinic Knoxville to identify sources capable of providing Contract Ambulance Services. Contractor will be required to furnish all labor, materials, insurance, licenses and equipment to provide safe and comfortable ambulance litter/stretcher transportation on an as-needed basis to beneficiaries of the Tennessee Valley Healthcare System (TVHS) Medical Centers at Nashville and Murfreesboro, TN and our associated Community Based Outpatient Clinics (CBOCs) in Tennessee and Kentucky. Services to be provided seven (7) days a week, as detailed further below, including holidays, to and from locations (i.e. clinics, ward locations, inpatient floors, ICU, patient homes, hospitals, pre-approved fee basis appointments) designated by the VA Medical Center (VAMC), Nashville or Murfreesboro Beneficiary Travel Office, in accordance with all terms, conditions, provisions and schedules set forth in this solicitation. Contractor shall assist the patient as needed to access the vehicle for transportation. This may include, but is not limited to bed to stretcher or stretcher to bed transfer when necessary. The applicable NAICS Code is 621910 (Ambulance Services) with a size standard of 7.0 million. This notice is for MARKET RESEARCH PURPOSES only and does not constitute a Request for Quote or Request for Proposal; and, it is not considered to be a commitment by the Government nor will the Government pay for information solicited: no basis for claim against the Government shall arise as a result from a response to this Sources Sought or Government use of any information provided. At this time no solicitation exists; therefore, DO NOT REQUEST A COPY OF THE SOLICITATION. The government reserves the right to consider any arrangement as deemed appropriated for this procurement. It is the VA MidSouth Healthcare Network (VISN 9) intent to issue a separate solicitation to be synopsized on the FedBizOpps website at https://www.fbo.gov. It is the potential offer's responsibility to monitor this avenue for release of any future solicitation or synopsis. The contractor shall furnish all labor, equipment, materials, transportation and supervision necessary to meet the requirements that will be listed in the specifications contained in the actual solicitation. ANY INTERESTED FIRM CAPABLE OF PROVIDING THESE SERVICES IS REQUESTED TO RESPOND VIA E-MAIL TO: ROY ROSSIGNOL AT ROY.ROSSIGNOL@VA.GOV NO LATER THAN 4:30 P.M., CENTRAL STANDARD TIME FEBRUARY 10, 2011 WITH THE FOLLOWING DOCUMENTATION: 1. Name of Company and Address, DUNS Number 2. Point of Contact and Phone number 3. Business size (small, veteran-owned small business, service-disable small business, HUBZone, 8(a), Small Disadvantaged business, woman owned or large business) (Small Business status to be certified by the VA and in conjunction with the SBA). 4. Capability Statement that demonstrates the ability to meet the requirements to support requirements for services required at the Mid South Healthcare Network for Tennessee Valley Healthcare System (TVHS). 5. Evidence of experience in work similar in type, scope, and complexity to include contract numbers, project titles, and dollar amounts, points of contact and telephone numbers, and statement of the scope and breathe of the projects. Interested parties shall furnish this information for at least three (3) contracts for the prime contractor, ongoing or completed within the last three years, for like or similar services, held with the VA, other Government agencies or private industry. 6. Corporate affiliations; potential joint venture partners, teaming partners, and or major subcontractors or (prime) information must be provided with your information. 7. Contractor locations that will support the requirement.. 8. A positive statement of your intent to submit a quote for this solicitation as a prime contractor COMPETITION AND SET-ASIDE DECISIONS MAY BE BASED ON THE RESULTS OF THIS MARKET RESEARCH. FAILURE TO SUBMIT INFORMATION IN SUFFICIENT DETAIL MAY RESULT IN CONSIDERING A COMPANY AS NOT A VIABLE SOURCE. Interested parties must be registered in the Central Contractor Registry (CCR) as prescribed in FAR Clause 52.232-33. CCR can be obtained by accessing the Internet at www.ccr.gov or by calling 1-888-227-2423. There is potential for either a single or multiple awards to be made on this contract. Awarded Contractor(s) must be able to meet the timeliness response requirements to and from the awarded location(s), with the majority of ambulance runs at the CBOCs being unscheduled emergent transports to the VAMC or a local community hospital when medically indicated. The Local Mileage Limits (LML), for the purpose of this requirement, is defined as mileage within a 40-mile radius in any direction, of the Nashville VA Medical Center, located at 1310 24th Avenue South, Nashville, TN and a 40-mile radius in any direction of the Alvin C. York VA Medical Center, located at 3400 Lebanon Pike, Murfreesboro, TN; or when transportation involves any of the CBOCs in Tennessee or Kentucky, as the pick-up or destination point, the LML is defined as mileage within a 40-mile radius in any direction of that VA CBOC. Any trip within the appropriate mile radius will be paid the applicable per trip rate. Any trip exceeding the mile radius will be paid the applicable per trip rate plus the mileage rate for any miles to the destination that exceeds the LML. The contractor will not be paid for the mileage traveled within the mile LML. Mileage will be based on the latest edition of readily available online commercial mileage calculator Rand McNally for online driving directions. No payment will be made for the contractor s mileage to the pickup location or return to dispatch station. AMBULANCE REQUIREMENTS: Seven (7) vehicles shall be required for services within the Nashville metro service area between the hours of 6:00 am and 6:00 pm, Monday through Friday to include all Federal Holidays. Three (3) vehicles shall be required for services within the Nashville metro service area between the hours of 6:00 pm and 6:00 am, Monday through Friday to include all Federal Holidays and twenty-four (24) hours per day on weekends, to include all holidays. Two (2) vehicles shall be required for services at locations defined to be outside the Nashville metro service area between the hours of 6:00 am and 6:00 pm, Monday through Friday to include all Federal Holidays. One (1) vehicle shall be required for services at locations defined to be outside the Nashville metro service area between the hours of 6:00 pm and 6:00 am, Monday through Friday to include all Federal Holidays and twenty-four (24) hours per day on weekends, to include all holidays. This vehicle requirement is defined as ambulances in use or in route exclusively for Nashville VAMC patients. All ambulances shall meet the standards of KKK-A-1822E dated June 01, 2002. Ambulance proposed to be furnished under this contract shall meet the specification enumerated in KKK-A-1822E dated June 01, 2002 including but not limited to the following for adults: (1) IV supplies, including pumps when required (2) Monitors, defibrillators, mounting brackets (3) First aid kit with band-aids, gauze, elastic bandages, sterile gauze pads, triangular bandages, cleansing wipes, tape scissors, eye pads, ammonia inhalants, sterile gloves, etc. (4) Two-way radio or portable phone (5) Face masks/nasal cannulae (6) Splints (7) Blood tubes (8) Oxygen. Expected to be needed on 10% of orders. The ambulance (emergency medical care vehicles) under the terms of this contract shall be licensed and meet the minimum vehicle requirements for basic life support as mandated by the Tennessee Department of Public Health and Department of Transportation pertaining to ambulance service. MISCELLANEOUS AMBULANCE REQUIREMENTS: The Department of Veterans Affairs (VA) will not be responsible for providing items that normally would be used or required for comfort, safety, or necessity of the patients being transported. These items include but or not limited to wrist restraints, disposable gloves, large pampers, linens, blankets, bedpans, urinal, emesis basins, pillows, and all other items normally stocked in ambulances. ATTENDANTS: A. A record of each employee as to character, mental, and physical capabilities of performing the duties of an ambulance driver or attendant must be maintained and made available for inspection upon request. An additional record of each employee showing that the employee performing transportation service for or coming in contact with patients of the Nashville VAMC is screened and found to be free of any infectious or communicable diseases on an annual basis. Qualifications of the attendants shall be submitted to the Contracting Officer prior to award. The COTR reserves the right to obtain copies of all qualification records for Nashville VAMC local files. New driver records should be made available to the Contracting Officer and/or COTR prior to commencement of the individual transporting VA patients. B. Contractor may allow, upon authorization from an authorized official of the VAMC, a VA clinician, relative, or other appropriate person to accompany the veteran at no extra charge. C. Only employees of the Contractor, who meet the qualifications of (see below) Emergency Medical Technician (EMT) or a Paramedics below shall be allowed to accompany the attendant/driver on trips provided for the Nashville VAMC. D. For airport trips, attendant shall remain with the patient until patient safely boards aircraft. It is estimated that approximately less than 1 trip per quarter will be to an airport. E. Psychiatric patient transports require one medical attendant, not the driver, to be fully dedicated to patient care during the transport. EMERGENCY MEDICAL TECHNICIAN (EMT): Contractor Emergency Medical Technicians (EMTs) who perform emergency services on ambulances servicing the Nashville VAMC and CBOCs shall have the qualifications: (1) EMTs must have completed training in accordance with the standards published by the Tennessee Department of Public Health Department of Health and Human Services. Such training programs must be acceptable under the regulating requirements for local EMS Systems supported by DHHS under PL 93-154. (2) The equivalent training program successfully completed by the EMT must be submitted to the Contracting Officer for appropriate action/ review with the bid proposal. (3) The EMT must be certified, licensed or otherwise officially recognized by the Local, State, or Regional Government or public entity where the emergency ambulance service is operated, or by which it is governed. Certificate shall be submitted with proposal. (4) The EMT must be enrolled periodically in refresher continuing education, or advanced training programs as required by the Local or State government entity in which the service is rendered to veterans, but in no instance shall this be less frequently than every two years. Such refresher training must be equivalent of that developed by the Department of Transportation, National Highway Safety Administration. Proof of such refresher training must be submitted to the Contracting Officer when updated periodically as required by the governmental entity. PARAMEDICS: Paramedics providing emergency services on ambulances servicing the Nashville or Murfreesboro VAMC and CBOCs shall have the following qualifications: (1) Complete training in accordance with the standards published by the Tennessee Department of Public Health to be licensed in the state of Tennessee. (2) The paramedic must be certified, licensed or otherwise officially recognized by the local, state, or regional government or public entity where the emergency ambulance service is operated, or by which it is governed. Proof of such certification must be submitted to the Contracting Officer (3) The paramedic must be enrolled periodically in refresher continuing education, or advanced training programs as required by the local or state government entity in which the service is rendered to veterans, but in no instance shall this be less frequently than every three years. Such refresher training must be equivalent to that developed by the Department of Transportation, National Highway Safety Administration. Proof of such refresher training must be submitted to the Contracting Officer for appropriate action / review as required by governmental entity It is the potential offeror's responsibility to monitor FedBizOpps website at https://www.fbo.gov for release of any future solicitation that may result from the sources sought.
