Description
The Portland, Oregon Veterans Administration Medical Center requires the services of an Occupational and Employee Health specialty walk-in clinic with the following capabilities: 1)Pre-employment and recurring physical exams that include: "Review of history "Review job description and at end of exam evaluate whether candidate meets essential functions "Measure height and weight "Vision test including near, far, color and depth perception "Blood pressure, pulse, temperature "Examine eyes, ears, nose and throat, thyroid "Neurological examination with Romberg "Respiratory examination "Gastrointestinal examination "Musculoskeletal examination including back and extremities "Evaluate flexibility and range of motion The services must be available on a walk-in basis during normal business hours, Monday through Friday. Clinic(s) are needed in the Portland, Oregon metropolitan area. The need for these services will be ongoing, although usage will fluctuate from month to month, based on examination requirements. Based upon current experience, the estimated annual volume of examinations is as follows: Physical Exam Level 3349 (Basic non-invasive functional evaluation and physical exam including height, weight, medical history, vision and hearing.) Physical Exam Level 436 (All components of Level 3 plus on-the-job job injury or illness initial evaluation and report.) DOT physicals58 Audiometric testing 429 (Testing with ANSI certified equipment and evaluated by a certified Audiologist) Vision432 OSHA audiometric tests150 (OSHA Hearing Conservation Program) The anticipated start date of this contract is April 1, 2011. The contract will be for an initial period of one year, ending March 31, 2012. Multiple extension options are provided through fiscal year 2016. This solicitation will result in a Firm-Fixed Price, non-personal services healthcare contract. North American Industry Classification System (NAICS) Code is 621399, size standard of $7.0 million.
