Description

Solicitation Text Standard Form 18 RFQ Letter The National Hansen's Disease Programs (NHDP) has a requirement for Orthotic Technician Services. Services are required 21 hours per week for one year from date of award, plus options for two additional years. Required services include (1) construction, modification and repair of custom molded sandals and modification and repair of prefabricated sandals; (2) modification of commercially-available shoes and repair both commercially available and custom shoes; (3) provide assistance to therapists with construction of foot models from various molding medium and adjustment of foot models as needed; (4) construction and modification of total contact orthoses; (4) adjustment of belts and prosthetic harnesses; (5) consultation with therapist on patient cases; (6) maintain records of work orders; (7) assist in maintenance of inventory by tracking materials and supplies used in the shoe shop; (8) safely store materials utilized in the shoe shop; (9) assist in education and training of visiting health care providers in sandal construction, shoe modification, construction of foot models and adjustments and construction of total contact orthoses; (10) provide assistance to Occupational and Physical Therapy departments in maintaining and fabricating a stock of monofilaments, cut out felt and other frequently used clinical items and assist in transfer of patients to/from department. This is a 100% small business set-aside, restricted to small businesses eligible to receive award. The RFP will be posted at the Government website at www.fbo.gov under HHS, Health Resources and Services Administration, BPHC/National Hansen's Disease Programs, Posted Dates on February 2, 2011. It is the offeror's responsibility to monitor the site for amendments to the solicitation. The NAICS code for this requirement is 339113 Surgical Appliance and Supplies Manufacturing. Solicitation will be conducted under Federal Acquisition Regulation (FAR) Part 12 and 13. The closing date for receipt of proposals will be approximately thirty (30) days after issuance/posting of the RFP.
