Box 149971 Austin TX 78714-9971 ACKNOWLEDGMENT OF AMENDMENTS: The offeror acknowledges receipt of amendments to the Solicitation numbered and dated as follows: AMENDMENT NO DATE ______________________________ _____________ ______________________________ _____________ ______________________________ _____________ Department of Veterans Affairs (VA) Veterans Health Administration Healthcare Recruitment and Retention Office Diversity Student Programs STATEMENT OF WORK (SOW) 1.0Introduction The VHA Healthcare Retention and Recruitment Office (HRRO) provides funding and oversight for several student training programs and internships. These internships offer college and graduate-level students on-the-job experience in a variety of clinical and administrative career areas. In Executive Order 13171 of October 12, 2000, it was noted that Hispanics remain underrepresented in the Federal workforce so the President affirmed ongoing policies and recommendations to eliminate the underrepresentation, to include providing a plan for recruiting Hispanics that creates a fully diverse workforce for the agency in the 21st century. It provides outstanding Hispanic Association of Colleges and Universities students the opportunity to gain valuable and personally rewarding work experiences with the nation's largest integrated health care system. Moreover, it affords them assignments in locations where they normally would not have the opportunity to go. Through these work experiences, the students may gain credit in fulfilling their academic requirements. In addition, VA is given a valuable opportunity to enhance the student's awareness and perceptions of VA as an employer of choice. These student career work experiences help lay the foundation for future VA employment and increase the likelihood of improving representation among VA's Hispanic population. VA is committed to employ a work force that mirrors the minority we serve. 2.0Scope of Work The Department of Veterans Affairs (VA) will provide the contractor with copies of documents that the Government is required to provide. The contractor shall request other government documentation deemed pertinent to the work accomplished directly from the government officials with whom the contractor has contact. The contractor shall consider the Program Manager/Contracting Officer's Technical Representative as the final source for need government documentation when the contractor fails to secure the documents by other means. The contractor is expected to use common knowledge and resourcefulness in securing all other reference materials, standard industry publications, and related materials that are pertinent to the work. The contractor shall maintain frequent communications with designated Veterans Affairs (VA) staff to conduct work in progress reviews. Reproduction of hand-out material (if applicable) will be forwarded to HRRO to reproduce. The VHA Healthcare Retention and Recruitment Office (HRRO) seeks to recruit highly-qualified (minimum 3.0 GPA on a 4.0 scale), Hispanic-American student interns through the wide array of resources available to a Hispanic-American serving organization. After placement, students are able to receive on-going training in the skill areas related to their career field. 3.0Period of Performance The period of performance will begin from the date of award through March 1, 2011 plus one (12) month option. 4.0Type of Order This will be awarded as a firm-fixed price. 5.0Place of Performance 6.0Specific Mandatory Tasks and Deliverables The contract will be for delivery of a student intern for each HRRO-designated position at VA Medical Centers throughout the United States. Task 1: Provide highly-qualified (minimum 3.0 GPA on a 4.0 scale) student candidates for student trainee temporary, summer positions at participating VA Medical Centers nationwide. The students must be: (1) Enrolled in a degree-seeking program, (2) Completion of the freshman year of college before the internship begins, (3) Authorized to work in the United States by law, (4) A United States citizen and (5) Fluent in English (oral and written). This includes innovative marketing techniques, active recruitment and screening of applicants. Methods of recruitment are to include the following: Sub-Tasks: a.visits to Hispanic Serving Institutions (HSIs) b.Responds to student and agency inquiries. c.Web-based marketing such as informational web pages, etc. d.Receives, screens, processes, and matches student applications with internship assignments provided by Program Manager. Task 2: Ensure the overall experience meets the interns' professional, academic and personal needs. Sub-Task: a.Collaborate and coordinate with Internship Program Managers at VA Medical Centers in determining the type of work experience that will be attained and providing candidates with similar interests. b.Provide round-trip airfare to and from the internship site. c.Provide a listing of safe, affordable, and furnished short-term housing near public transportation d.Offer student support services such as career counseling and other developmental activities. Timeline of Deliverables: 1.Contractor shall contact Program Manager within five business days after award. 2.Contractor shall match students upon Program Manager's approval of internship sites. The Program Manager is solely authorized to approve VA internship sites. 7.0Changes to the Statement of Work a. Only the Contracting Officer is authorized to make changes to this SOW; and the Contracting Officer will authorize such changes only through written correspondence. The COTR will keep a copy of each change in a project folder along with all other products of the project. The contractor shall bear any and all costs incurred by the contractor through the actions of parties other than the Contracting Officer. b. The Contracting Officer Technical Representative (COTR) and VA Program Manager (PM) or any other individual (including VA or other Government agency employees or employees of other contractors) other than the VA Contracting Officer are not authorized to make any changes of a contractually binding nature to the period of performance, funding and/or any other terms and conditions of any award or order resulting from this solicitation. The COTR Appointment Letter stating the COTR's authority and limitations is attached to this SOW and is incorporated into the terms and conditions of any award or order resulting from this solicitation. 8.0Travel There is no foreseeable need for travel to or from the HRRO facility. 9.0Key Personnel The contractor shall assign a single individual to serve as the primary point of contact for the HRRO Program Manager. Any changes of key personnel shall require the Contracting Officer's and Program Manager's approval. 10.0Government and Contractor Responsibilities a. The Department of Veterans Affairs (VA) will provide the contractor with copies of documents that the Government is required to provide. b. The contractor shall request other government documentation deemed pertinent to the work accomplishment directly from the government officials with whom the contractor has contact. The contractor shall consider the PM/COTR as the final source for needed government documentation when the contractor fails to secure the documents by other means. The contractor is expected to use common knowledge and resourcefulness in securing all other reference materials, standard industry publications, and related materials that are pertinent to the work. c. The contractor shall maintain frequent communications with designated Veterans Affairs (VA) staff to conduct work in progress reviews. d. Reproduction of hand-out material (if applicable) will be forwarded to HRRO to reproduce. 11.0 Other Personnel Considerations a. Personnel assigned by the contractor to the performance of work on this order shall be acceptable to VA in terms of personal and professional conduct and technical knowledge. Should the assignment to this order of any person by the contractor be deemed to conflict with the interests of VA, or in the event performance is deemed to be unsatisfactory at any time during the life of the contract, the Contracting Officer may notify the contractor and request the person be immediately removed from the assignment. The reason for removal will be documented and a request to receive personnel replacement within three (3) business days of the notification will be made. Replacement personnel qualifications shall be equal to or greater than those of the personnel being replaced. Employment and staffing difficulties will not be justification for failure to meet established schedules. b. Due to the technical nature of this contract, the contractor is required to maintain the competency level put forth in the original proposal submitted. Should the contractor, at any time during the performance of this contract propose a change in that skill set, the PM and COTR must first review and approve the change before it is implemented. The contractor shall submit a resume of qualifications and the Contractor Personnel Change Control form to the PM and COTR for all additional direct employees proposed for the project. The PM and COTR will approve all contractor employees prior to bringing on duty via Contractor Personnel Change Request Form (Attachment B). If, at any time from date of award to the end of the contract, contractor personnel are no longer available, the VA will approve the qualifications of proposed replacement personnel and will reject individuals who do not meet the qualifications set forth herein. All contractor employees are subject to immediate removal from performance of this contract when they are involved in a violation of the law, VA security, confidentiality requirements, and/or other disciplinary reasons. c. The VA will not be liable for further payment of any contractor employee that VA requires the contractor to remove from the performance of this contract. VA's payment liability, for unsuitable contractor employees, ends five working days after contractor's receipt of "removal request." 12.0Invoicing Acceptance and Invoicing - As the Contractor completes tasks and provides deliverables, they shall submit electronically to the VA PM/COTR at SDDContract@med.va.gov a certificate of completion for each deliverable. The VA PM/COTR will inspect, or cause to be inspected, and certify acceptability, of all services or products delivered under this contract before processing applicable invoices for payment. The VA PM/COTR will certify to the CO that the services or products delivered meet the defined quality standards. The CO shall make formal acceptance or rejection of all deliverables in writing. If rejected, the reasons for rejection shall be documented in writing and provided to the Contractor for correction. The Contractor may invoice for priced deliverables immediately upon acceptance. A sample of the detail sheet and Microsoft Excel template of Standard Form 1034 are available from VA OIT VHIT Budget & Contract Administration Service. To request a copy, please send an e-mail request to SDDContract@med.va.gov. Invoice discrepancies shall be submitted within 90 days from the last day of the monthly invoice period. The Government shall not accept any invoices for work more than 90 days old. The Contractor recognizes that any work more than 90 days old (from the last day of the monthly invoicing period) shall not be invoiced by the Contractor and the Government shall not be liable to make payment due to Contractor's failure to conform to the invoice requirements specified. Invoices shall specify the monthly billing period, project title, purchase order number, contract number and supporting documentation. Contractor key project personnel shall be in coordination with the VA PM/COTR. Changes in key project personnel shall not be made without prior coordination with the VA PM/COTR. Therefore, the contractor's initial quote should contain the contractor's best terms from a cost or price and technical standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. The Government may reject any or all quotes if such action is in the public interest; accept other than the lowest quote; and waive informalities and minor irregularities in quotes received. B.Quote Due Date: Contractor must complete and return all information prior to February 8, 2011, 1:00 p.m. EST, in order to be considered for award. The contractor shall submit its quote electronically via e-mail to the Contracting Officer. The e-mail address is adeitra.jimmison@va.gov. C.Content: The Government will be evaluating quote on the basis of the material presented in the written quote. The contractor's quote should contain: 1.Technical Response: The technical response should be written in plain language and tailored specifically to the tasks listed in the solicitation. The contractor should explain how they will be handling each task and supporting documentation including (1) Qualifications of contractor and SME to perform the tasks required to ensure accomplishment of contract objectives. 2.Price: The number of pages for the price portion of the quote is not limited, but shall include the following information at a minimum: "A detailed quote applying the labor categories, labor rates and labor hours for the requirements stated in this SOW. The quote shall price each individual task. Each task shall be broken by labor hours, labor rates, and travel and other direct costs. "The quote shall price non-travel Other Direct Costs on a Fixed-Price basis. 3.Resumes: N/A. F.VETERANS INVOLVEMENT EVALUATION APPROACH In accordance with VAAR 852.215-70, Service-Disabled Veteran-Owned (SDVOSB) and Veteran-Owned Small Business (VOSB) Evaluation Factors, the Government will assign evaluation credit for an Offeror (prime contractor or team member) which is a SDVOSB or a VOSB. Non-veteran Offerors or Teams proposing to use SDVOSBs or VOSBs as subcontractors will receive some consideration under this evaluation Factor VOLUME V - VETERANS INVOLVEMENT FACTOR. i.For SDVOSBS/VOSBs: In order to receive credit under this Factor, an Offeror shall submit a statement of compliance that it qualifies as a SDVOSB or VOSB in accordance with VAAR 852.215-70, Service-Disabled Veteran-Owned and Veteran-Owned Small Business Evaluation Factors. Offerors are cautioned that they must be registered and verified in Vendor Information Pages (VIP) database (http://www.VetBiz.gov). ii.For Non-SDVOSBs/VOSBs: To receive some consideration under this Factor, an offeror must state in its quote the names of SDVOSB(s) and/or VOSB(s) with whom it intends to subcontract, and provide a brief description and the approximate dollar values of the proposed subcontracts. Additionally, proposed SDVOSB/VOSB subcontracts must be registered and verified in Vendor Information Pages (VIP) database (http://www.VetBiz.gov) in order to receive some consideration under the Veteran's Involvement Factor. SECTION C - CONTRACT CLAUSES C.1 VAAR 852.273-75 SECURITY REQUIREMENTS FOR UNCLASSIFIED INFORMATION TECHNOLOGY RESOURCES (Interim - October 2008) (a) The contractor and their personnel shall be subject to the same Federal laws, regulations, standards and VA policies as VA personnel, regarding information and information system security. These include, but are not limited to Federal Information Security Management Act (FISMA), Appendix III of OMB Circular A-130, and guidance and standards, available from the Department of Commerce's National Institute of Standards and Technology (NIST). This also includes the use of common security configurations available from NIST's Web site at: http://checklists.nist.gov (b) To ensure that appropriate security controls are in place, Contractors must follow the procedures set forth in "VA Information and Information System Security/Privacy Requirements for IT Contracts" located at the following Web site: http://www.iprm.oit.va.gov/docs/Security_and_Privacy_Requirements_for_IT_Contracts_Attachment.pdf (End of Clause) C.2 VAAR 852.273-76 ELECTRONIC INVOICE SUBMISSION (Interim - October 2008) (a) To improve the timeliness of payments and lower overall administrative costs, VA strongly encourages contractors to submit invoices using its electronic invoicing system. At present, electronic submission is voluntary and any nominal registration fees will be the responsibility of the contractor. VA intends to mandate electronic invoice submission, subject to completion of the federal rulemaking process. At present, VA is using a 3rd party agent to contact contractors regarding this service. During the voluntary period, contractors interested in registering for the electronic system should contact the VA's Financial Services Center at http://www.fsc.va.gov/einvoice.asp. C.3 VAAR 852.237-70 CONTRACTOR RESPONSIBILITIES (APR 1984) The contractor shall obtain all necessary licenses and/or permits required to perform this work. He/she shall take all reasonable precautions necessary to protect persons and property from injury or damage during the performance of this contract. He/ she shall be responsible for any injury to himself/herself, his/her employees, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by his/her employees fault or negligence, and shall maintain personal liability and property damage insurance having coverage for a limit as required by the laws of the State of. Further, it is agreed that any negligence of the Government, its officers, agents, servants and employees, shall not be the responsibility of the contractor hereunder with the regard to any claims, loss, damage, injury, and liability resulting there from. (d) Comptroller General Examination of Record. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.
